摘要 目的:探讨卡介苗多糖核酸与308 nm准分子激光对白癜风患者外周血Th17细胞亚群的影响。方法:选取2013年1月-2015年1月本院皮肤性病科白癜风患者192例作为研究对象,采用随机数字表法将其分为研究组与对照组,每组96例,对照组采用308 nm准分子激光治疗,研究组在对照组基础上采用卡介苗多糖核酸治疗,采用流式细胞仪检测外周血Th17细胞亚群变化,比较两组临床疗效及不良反应发生情况。结果:治疗后,研究组治疗总有效率明显高于对照组,其中同组进展期白癜风患者治疗总有效率明显高于稳定期患者,比较差异均有统计学意义(P<0.05)。治疗前,两组外周血Th17细胞亚群比例比较,差异无统计学意义(P>0.05);治疗后,观察组外周血Th17细胞亚群比例明显低于治疗前与对照组,比较差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:308 nm准分子激光对进展期白癜风患者的临床疗效较为显著;卡介苗多糖核酸通过提高机体免疫功能,从而改善白癜风患者的临床疗效,且药物安全性较高,值得临床推广应用。 Objective:To investigate the effect of BCG-polysaccharide nucleic acid and 308 nm excimer laser on peripheral blood Th17 cell subsets of vitiligo patients.Method:192 cases of vitiligo patients in STD Dermatology of our hospital from January 2013 to January 2015 were selected as the research objects.Using the random number table method,all patients were divided into study group and control group,with 96 cases in each group.The control group was treated with 308 nm excimer laser,the study group was treated with BCG nucleic acid on the basis of the control group.Peripheral blood Th17 cell subsets were detected by flow cytometry.The clinical curative effect and the incidence of adverse reactions were compared between the two groups.Result:After treatment,the total effective rate of the study group was significantly higher than that of the control group,the total effective rate of advanced stage was significantly higher than that of stable stage in the same group,the differences werestatistically significant(P〈0.05).Before treatment,the difference in peripheral blood Th17 cell subsets between the two groups was not statistically significant(P〉0.05).After treatment,the peripheral blood Th17 cell subsets of the observation group was significantly lower than pre-therapy and that of the control group,the differences were statistically significant(P〈0.05).There was no statistically significant difference in the incidence rate of adverse reactions between the two groups(P〉0.05).Conclusion:308 nm excimer laser has more significant clinical curative effect for advanced vitiligo.BCG nucleic acid improves the clinical curative effect by improving the organism immune function for vitiligo,its drug safety is high,which is worthy of clinical application.
