The sleep architecture in patients with multiple system atrophy

摘要 目的:探讨多系统萎缩(MSA)患者的睡眠结构改变。方法:选择MSA患者15例。同时纳入年龄、性别匹配的健康对照组19例。利用日本光电32导9200K脑电图机,所有患者均完成多导睡眠生理监测(PSG),记录脑电图、眼球运动、下颌和肢体肌电活动、心电图、经鼻气流、胸腹部呼吸运动、血氧、鼾声,并录像监测患者的行为。使用Polysmith软件分析呼吸、运动等相关指标,并视觉评估睡眠分期。结果:MSA组患者具有典型临床表现。和对照组相比,MSA组总体睡眠时间缩短、睡眠效率减低、Ⅱ期睡眠时间减少、低通气指数增高、周期性腿动(PLM)指数增高。全部MSA患者均呈现快速眼动期骨骼肌张力不消失,其中5例(30%)患者伴有夜间活动和言语。结论:MSA患者出现睡眠结构紊乱、呼吸紊乱以及PLM指数增高,并出现快速眼动期睡眠行为障碍(RBD),RBD可能具有提示诊断MSA的作用。 Objective:To investigate the changes of sleep structure in patients with multiple system atrophy (MSA).Methods:Fifteen patients with MSA were included,19 age and sex matched healthy people recruited as control.All participants were video-mon'itored with recordings of electroencephalography,electrooeulography and eleetromyography on the chins and limbs,electrocardiography,nasal airflow,thoracic and abdominal respi- ratory movement,oxygen and snore.The sleep architecture and other parameters were analyzed on the software of Polysmith and analyze sleep stages through vision.Results:Compared to healthy control,MSA group was found to have shorter total Sl^eep time and stage Ⅱtime,lower sleep efficieney.Meanwhile,hypopnea and periodic limb movement (PLM)indexlincreased.All patients with MSA had loss of atonia and five (30%)of them were recorded of night behaviors and talks.Conclusion:The MSA patients have sleep structure disorder,respiratory disorder,abnormal PLM and rfipid eye movement sleep behavior disorder.
