不同手术方式治疗甲沟炎的临床效果对比

Comparison of clinical effects of different operation methods in treatment of paronychia

目的分析不同手术方式治疗甲沟炎的临床效果。方法随机抽取2012年4月~2014年11月诊治的90例甲沟炎患者,并将其分成三组。凹陷型甲沟炎患者使用甲床部分切除再加上趾侧或者趾前的梭形切皮整形的方法进行治疗;而对扁平型、嵌甲型甲沟炎患者使用甲床的部分切除的方法进行治疗,分析三组的治疗效果。结果扁平型患者的治疗的痊愈率显著高于嵌甲型、凹陷型患者,两者间的差异具备统计学的意义（P〈0.05）;凹陷型患者的治疗的痊愈率显著高于嵌甲型患者,两者间的差异具备统计学的意义（P〈0.05）。扁平型患者的治疗的复发率显著低于嵌甲型、凹陷型患者,两者间的差异具备统计学的意义（P〈0.05）;凹陷型患者的治疗的复发率显著低于嵌甲型患者,两者间的差异具备统计学的意义（P〈0.05）。结论扁平型、嵌甲型的甲沟炎使用甲床部分切除的方法进行治疗就可以彻底解决嵌甲的情况。而凹陷型甲沟炎的患者在实施甲床部分切除至后,还需要进行趾前或者趾侧梭形切皮整形,才能彻底治愈甲沟炎,提高患者的生活质量。

Objective to analyze the clinical effects of different operation methods in treatment of paronychia. Methods 90 cases of paronychia treated from April 2012 to November 2014 and were randomly selected, and divides them into three groups. Depression type paronychia patient use nail bed partial resection plus on toe side or front toe spindle cutting skin shaping method to carry on the treatment; and patients with ingrown toenails and flat type paronychia use nail bed partial excision method treatment, and analysis the therapeutic effect of the three groups. Results the cure rate of the patients with flat type was significantly higher than that in the patients with type A and depression. The difference has statistical significance（P〈0.05）; The cure rate of onychocryptosis type were significantly higher than that of the patients with depression. The difference between them was statistically significant（P0.05）. The recurrence rate of patients with flat type was significantly lower than that in patients with type A and depression, and the difference was statistically significant（P0.05）. Onychocryptosis patients recurrence rate was significantly lower than that in patients with depression type, the difference has statistical significance（P0.05）. Conclusion the patients with flat type, onychocryptosis type use nail bed partial resection method can completely solve the ingrown toenail disease. Hollow type paronychia patient after the implementation of the partial excision of nail bed, still need toe or toe side spindle shaped skin incision surgery, it can thoroughly cure paronychia,and improve quality of life.
