儿童急性鼻窦炎并发眶蜂窝织炎14例临床分析

Clinical analysis of 14 cases of acute sinusitis in children complicated with orbital cellulitis

目的:探讨儿童急性鼻窦炎并发眶蜂窝织炎的临床特点及诊治方法。方法:回顾性分析14例因急性鼻窦炎引起的眶蜂窝织炎患儿的临床资料,根据病史、体格检查、影像学检查及手术记录等了解该病的临床特点和诊治方法。结果:13例患儿应用抗生素保守治疗1周后效果欠佳,除1例自动离院失访,其余均经手术治疗后治愈。有1例8岁患儿从眼科转入我科即右眼失明,虽经手术治疗,但视力仍未改善。结论:急性鼻窦炎并发眶蜂窝织炎起病急、进展快,如及早诊断、积极治疗可有效预防眼球运动障碍和视力损害,提高治愈率。

Objective：To investigate the clinical characteristics and treatment methods of acute sinusitis in children with orbital cellulitis.Method：Fourteen cases in our hospital of acute sinusitis in children which causes orbital cellulitis were collected.The history,physical examination,imaging examination,and surgical records were analyzed to understand the clinical characteristics and treatment of the disease.Result：Thirteen caseshad poor efficacy after been treated with antibiotics.Excepted 1case automatic left the hospital,the rest were cured by surgical treatment.The cure rate was 100%.One 8years old child transfered in department of rhinology from department of ophthalmology with his eye blind.Inspite of surgury,the vision didn＇t improve.Conclusion：Acute sinusitis in children with orbital cellulitis has a rapid onset and is rapidly spreading.Early diagnosis,active treatment can effectively prevent disorders of ocular movement,visual impairment and can improve the cure rate.
