内痔齿形套扎外痔剥离术治疗混合痔的临床观察

Clinical Observation on Method of Jugged Rubber Band Ligation for Internal Hemorrhoid and Dissection for External Hemorrhoid in Treatment of Mixed Hemorrhoids

目的:评价内痔齿形套扎外痔剥离术治疗混合痔的临床疗效。方法:将200例混合痔患者随机分为治疗组和对照组,每组100例。治疗组采用内痔齿形套扎外痔剥离术,对照组采用外剥内扎术治疗。评价两组的临床疗效,观察两组术后疼痛、创面渗血、创缘水肿及排便情况,比较两组患者的手术时间及创面愈合时间。结果:两组的总有效率均为100%,组间比较差异无统计学意义(P>0.05)。术后第2天,治疗组患者的创面渗血情况优于对照组(P<0.01);术后第7天,治疗组患者的疼痛、创面渗血情况均优于对照组(P<0.05,P<0.01)。术后第2、7、10天,治疗组患者的排便情况均优于对照组(P<0.01)。治疗组患者的手术时间及创面愈合时间较对照组均明显缩短(P<0.01)。结论:与外剥内扎术相比,采用内痔齿形套扎外痔剥离术治疗混合痔具有术后疼痛轻、并发症少、创面愈合时间短的优势。

Objective： To evaluate the curative effects of the method of jugged rubber band ligation for internal hemorrhoid and dissection for external hemorrhoid in the treatment of mixed hemorrhoids. Methods： Two hundred patients with mixed hemorrhoids were randomly divided into treatment group and control group,with 100 cases in each group. The patients in the treatment group were treated by the method of jugged rubber band ligation for internal hemorrhoid and dissection for external hemorrhoid,and the patients in the control group were treated by the method of external dissection and internal ligation. The clinical effect was evaluated,the postoperative pain,hemorrhage and edema of wound and cacation were observed,and the operation time and wound healing time were compared. Results： The total effective rates of both groups were 100%,with no statistical difference between the two groups（ P 0. 05）. On the second day after operation,the hemorrhage of wound in the treatment group was superior to the control group（ P 0. 01）; on the seventh day after operation,the pain and hemorrhage of wound in the treatment group were superior to the control group（ P 0. 05,P 0. 01）. On the second,seventh and tenth days after operation,the cacation in the treatment group was superior to the control group（ P 0. 01）. The operation time and wound healing time of the treatment group were shorter than those of the control group（ P 0. 01）. Conclusion： Compared with the method of external dissection and internal ligation,the method of jugged rubber band ligation for internal hemorrhoid and dissection for external hemorrhoid can reduce the postoperative pain and complications and shorten the wound healing time.
