心理综合干预对脑卒中患者情绪障碍及康复效果的影响

Effect of comprehensive psychological intervention on emotional disorder and rehabilitation in patients with stroke

目的探讨心理综合干预对脑卒中患者情绪障碍及康复效果的影响。方法选取2013年1月至2014年1月在本院住院治疗的脑卒中患者120例，采用随机数字表法分为观察组与对照组，根据脑卒中患者的不同疾病类型及心理特点，对照组给予常规护理，观察组在对照组的基础上给予心理护理，应用医院焦虑自评量表（SAS）和医院抑郁自评量表（SDS）对两组患者护理前后心理状况进行评价和比较，并在出院时比较两组患者的康复效果。结果经过6个月的护理干预，观察组患者SDS评分（42．3±6．4）及SAS评分（46．9±8．1）与对照组相比显著下降，差异有统计学意义（P〉0．05）。观察组患者通过6个月护理干预后，Barthel评分为（80．8±15．8）分，较对照组[（50．2±18．2）]分相比显著提高，差异有统计学意义（P〈0．05）。结论心理干预对脑卒中情绪障碍能起到很好的治疗作用，并促进了躯体功能的康复。

Objective To explore the effect of comprehensive psychological intervention on emotional disorder and rehabilitation in patients with stroke. Methods 120 stroke patients hospitalized at our hospital from January, 2013 to January, 2010, were randomly divided into an observation group and a control group. According to the different disease types and psychological characteristics of patients with stroke, the control group were routinely cared and in addition, the observation group were psychologically cared. Hospital anxiety self rating scale （SAS） and hospital anxiety self rating scale （SDS） were used to evaluated the psychological status of both groups before and after the nursing care; and the results were compared. The rehabilitation effect were compared between these two groups. Results After 6 months＇ nursing intervention, the scores of SDS and SAS were （42.3±6.4） and （46.9±8.1） in the observation group, which were significantly lower than those in the control group, with statistical differences（P 〉0.05）; the Barthel score was （80.8±15.8） in the observation group and was （50.2±18.2） in the control group, with a statistical difference （P 〈0.05）. Conclusions Psychological intervention plays a very good therapeutic role in the treatment of emotional disorders of stroke patients and can promote the patients rehabilitation of physical function.
