摘要
目的观察综合药学干预对慢性病患者的影响,探索基层医院临床药学服务模式。方法调查分析慢性病患者对疾病的认知度和用药依从性,制订针对性的药学干预措施,提高患者的药物治疗效果。结果通过以综合药学干预为形式的临床药学服务措施,强化了患者的健康意识,显著提高了用药依从性和疾病控制效果。结论延伸临床药学服务,强化综合药学干预,实施个体化用药指导,是提高慢性病用药依从性和治疗效果的有效途径;采用灵活多样的形式,建立可持续的长效机制,才能充分发挥临床药学服务的效果。
Objective To observe the effect of comprehensive pharmaceutical intervention on patients with chronic diseases,to explore the clinical pharmacy service mode in the primary hospital.Methods The cognition and compliance of the patients with chronic diseases were investigated and analyzed,the targeted pharmaceutical interventions were developed,and the effect of drug treatment in patients was improved.Results Clinical pharmacy service measures through the comprehensive pharmaceutical intervention strengthened the patient′ s health awareness,significantly improved medication compliance and disease control effect.Conclusion The extension services through clinical pharmacy strengthen the comprehensive pharmaceutical intervention,implement personalized medicine instruction,is an effective way to improve medication compliance and treatment effect of chronic diseases;adopt flexible and diverse forms,establish a sustainable long-term mechanism,to bring the effect of clinical pharmaceutical care to a full play.
