N9-N20峰间潜伏期在颈椎管内肿瘤的应用价值

摘要 目的探讨颈椎管内肿瘤患者中,正中神经短潜伏期体感诱发电位(MN-SLSEP)的N9-N20峰间潜伏期的变化特点及其与脊髓功能预后的关系。方法选取接受住院手术治疗的颈椎管内肿瘤22例作为试验组,同时配对选取年龄完全匹配的健康受试者22例作为对照组。两组均在术前3 d及术后1个月两个时间点接受体感诱发电位(SEP)检测。比较两组间N9-N20峰间潜伏期的差异及试验组术前后的变化。通过将N9-N20峰间潜伏期分为潜伏期延长(lat+)组和潜伏期正常(lat)组,进一步比较其与脊髓功能预后的关系。结果试验组术前N9-N20峰间潜伏期较对照组明显延长(t=3.053,P<0.05)。试验组术后N9-N20峰间期较术前显著缩短(t=2.198,P<0.05)。lat组的预后改善率(100.0%)明显高于lat+组(44.4%)(P<0.05)。结论 N9-N20峰间潜伏期是MNSLSEP的可靠分析指标,可用于颈椎管内肿瘤脊髓功能预后的判定。 Objective The changes of N9-N20 interpeak latency of median nerve short latency somatosensory evoked potential( MN-SLSEP) in patients with cervical intraspinal tumors are investigated,and its association with prognosis of spinal cord function is discussed.Methods Twenty-two patients suffering from cervical intraspinal tumors were enrolled into study group.Meanwhile,twenty-two age-matched volunteers were selected as control group.The somatosensory evoked potential( SEP) detection was performed at pre-operative 3 d and post-operative one month,respectively.The difference of N9-N20 interpeak latency value between two groups was compared.Comparative analysis of discrepancy was made between pre-operative and post-operative value.Study group was further divided into delayed latency group( lat + group) and normal latency group( lat group),and the relationship was explored between N9-N20 interpeak latency and prognosis of spinal cord function.Results Pre-operative N9-N20 interpeak latency of study group was significantly longer than that of control group( P〈0.05).Post-operative N9-N20 interpeak latency of study group was significantly shorter than that of pre-operative value( P〈0.05).Improvement rate of spinal cord function of lat group( 100.0%) was significantly higher than that of lat + group( 44.4%,P〈0.05).Conclusion N9-N20 interpeak latency is a dependable index to MN-SLSEP and can be used to determine the prognosis of cervical intraspinal tumors.
