雷珠单抗治疗视网膜静脉阻塞继发黄斑水肿的效果及安全性的系统评价

Effect and safety of the treatment of retinal vein occlusion secondary to macular edema

目的分析雷珠单抗治疗视网膜静脉阻塞继发黄斑水肿的效果及安全性。方法选取2013年7月～2015年3月视网膜静脉阻塞继发黄斑水肿患者92例,分为目标组与参照组,参照组患者给予曲安奈德玻璃体腔注射,目标组患者给予雷珠单抗玻璃体腔注射,并选取同期视网膜静脉阻塞继发黄斑水肿患者47例作为空白组,仅给予激光药物治疗,对比三组最佳矫正视力（best-corrected visual acuity,BCVA）、黄斑中心凹厚度（central macular thickness,CMT）、副作用。结果干预后参照组和目标组BCVA水平均明显提高、CMT水平均明显降低（P〈0.05）,目标组改善水平明显优于参照组（P〈0.05）。目标组和参照组BCVA水平、CMT水平均明显优于空白组（P〈0.05）。三组副作用发生率相似,组间无明显差异（P〉0.05）。结论雷珠单抗治疗视网膜静脉阻塞继发黄斑水肿跟曲安奈德均有效,但雷珠单抗治疗效果更显著,可有效减轻黄斑水肿症状,改善患者视力水平,且安全性较高,值得推广应用。

Objective To analyze and study the effect and safety of the treatment of retinal vein occlusion secondary to macular edema after retinal vein occlusion. Methods A total of 92 patients with macular edema secondary to retinal vein occlusion were selected from July 2013 to March 2015. The patients were divided into target group and control group. The control group was treated with the vitreous cavity injection of triamcinolone acetonide, the target group was treated with monoclonal antibody, and 47 patients with retinal vein occlusion were treated by laser therapy and selected as blank group. The BCVA, CMT and side effects were compared among the three groups. Results The levels of BCVA in the reference group and the target group was significantly improved, the level of CMT was significantly decreased（P〈0.05）, the improvement of the target group was significantly better than that in the reference group（P〈0.05）.The level of BCVA and CMT in the target group and the reference group was significantly better than that in the blank group（P〈0.05）. The incidence rate of side effects was similar in the 3 groups, there was no significant difference among the three groups（P〉0.05）. Conclusion Thunder efalizumab in the treatment of retinal vein obstruction secondary to macular edema with triamcinolone acetonide are effective, but lucentis treatment effect are more significant, can effectively alleviate the symptoms of macular edema, improve patient acuity levels and higher security, it is worth popularization and application.
