摘要
目的探讨在采用甘露醇和甘油果糖治疗急性脑血管患者时对其肾功能损害情况。方法整群选取并回顾性分析2013年7月—2015年7月在该院住院治疗的130例急性脑血管病患者,其中单独采用甘露醇脱水治疗的患者42例(甘露醇组),单纯采用甘油果糖脱水治疗的患者45例(甘油果糖组),采用甘露醇和甘油果糖联合脱水患者43例(联合组),对比三组患者在用药过程中对患者肾功能损伤情况。结果甘露醇组患者用药2周后,出现肾功能损害比例16.67%(7/42),查尿素氮为(24.29±6.17)mmol/L,肌酐为(228.84±30.77)μmo I/L,甘油果糖组出现肾功能损害比例4.44%(2/45),查尿素氮为(14.38±4.71)mmol/L,肌酐为(160.12±29.45)μmo I/L,联合组肾功能损害比例4.65%(2/43),查尿素氮为(15.19±5.54)mmol/L,肌酐为(170.63±32.84)μmo I/L。发现甘露醇组患者肾功能损害发生率、受损患者平均尿素氮以及平均肌酐均要高于其他两组,差异具有统计学意义(P<0.05)。结论甘露醇联合甘油果糖治疗急性脑血管病患者,脱水效果确切,且用药相对安全,能够减少对肾功能的损害。
Objective To discuss the damage to kidney function in patients with acute cerebrovascular disease treated with mannitol and glycerin fructose. Methods 130 cases of patients with acute cerebrovascular disease treated in our hospital from June 2013 to June 2015 were retrospectively analyzed, the mannitol group(n=42) were given only mannitol dehydration treatment, the glycerol fructose group(n=45) were given only glycerol fructose dehydration treatment, the combination group were given the combined treatment of mannitol and glycerol fructose, in the process of drug use,the damage to kidney function of the three groups was compared. Results After 2 weeks of drug use, kidney function damage ratio, urea nitrogen and creatinine were respectively 16.67%(7/42),(24.29±6.17)mmol/L and(228.84±30.77)μmo I/L,4.44%(2/45)in the mannitol group and were respectively(14.38±4.71)mmol/L and(160.12±29.45)μmo I/L in the glycerol fructose group and 4.65%(2/43),(15.19±5.54)mmol/L and(170.63±32.84)μmo I/L in the combination group, the incidence of kidney function damage,the average urea nitrogen and the average creatinine of patients with impaired in the mannitol group were higher than those in the glycerol fructose group and the combination group, and the differences were significant(P〈0.05). Conclusion The dewatering effect of mannitol and glycerol fructose in the treatment of patients with acute cerebrovascular disease is definite and the drug use is relatively safe, which can reduce the damage to kidney function.
