自体及异体输血对胃癌根治术患者炎性因子及免疫功能的影响 被引量:4

Effect of Autogeneic or Allogeneic Transfusion on Inflammatory Factor and Immunologic Function in Patients with Radical Operation for Carcinoma of Stomach

摘要 目的研究自体及异体输血对胃癌根治术患者炎性因子及免疫功能的影响。方法整群抽选2012年6月—2014年8月在该院接受胃癌根治术的早期胃癌患者96例,随机分为自体组(n=48)和异体组(n=48),异体组术中输注异体全血,自体组输入自体血,比较术前、后两组患者自然杀伤细胞(NK细胞)、白介素-2(IL-2)及T淋巴细胞亚群(CD3+、CD4+、CD4+/CD8+)水平。结果术后当天两组患者NK细胞、IL-2较术前显著下降(P<0.05);异体组术后当天IL-2较自体组显著较低(P<0.05);异体组术后7 d NK细胞较自体组显著较低(P<0.05);术后当天两组T淋巴细胞亚群各指标均显著下降(P<0.05);术后7 d异体组CD3+、CD4+、CD4+/CD8+水平较自体组显著较低(P<0.05)。结论胃癌根治术患者输注自体血对免疫功能抑制作用小,值得推广。 Objective To research the effect of autogeneic or allogeneic transfusion on inflammatory factor and immunologic function in patients with radical operation for carcinoma of stomach. Methods 96 cases of early gastric cancer patients receiving the radical operation for carcinoma of stomach in our hospital from June 2012 to August 2014 were selected and randomly divided into the autogeneic group and the allogeneic group with 48 cases in each, the allogeneic group were transfused with allogeneic whole blood, the autogeneic group were transfused with autologous blood, the natural killer(NK cells), proleulzin(IL-2)and T lymphocyte subsets level(CD3+、CD4+、CD4+/CD8+)of the two groups before and after operation were compared. Results The NK cells and IL-2 of the two groups at the same day after operation were obviously decreased compared with those before operation,(P〈0.05); the IL-2 at the same day after operation in the allogeneic group was lower than that in the autogeneic group(P〈0.05); the CD3+,CD4+and CD4+/CD8+levels on the seventh day after operation in the allogeneic group were obviously lower than those in the autogeneic group,(P〈0.05). Conclusion The infusion of autologous blood in patients with radical operation for carcinoma of stomach has a small inhibiting effect on the immunologic function,which is worth promoting.
