无创呼吸机辅助通气联合吗啡治疗急性左心力衰竭临床疗效分析

Clinical Curative Effect Analysis of Non-Invasive Ventilator Assisted Ventilation Combined with Morphine in Treatment of Acute Left Heart Failure

目的探讨无创呼吸机辅助通气联合吗啡治疗急性左心衰竭的临床疗效。方法整群选择该院2013年1月—2015年1月该院收治的急性左心衰竭患者92例,将患者随机分为治疗组、对照组,每组46例。对照组的患者采用无创呼吸机辅助通气治疗,实验组在对照组基础上联合吗啡进行治疗。对比两组患者治疗有效率,平均动脉压(MAP)、心率(HR)、呼吸频率(RR)、平均动脉压(Pa O2)等指标。结果治疗组:有效27例(58.70%),显效15例(32.61%),总有效率91.31%;对照组:有效15例(32.61%),显效17例(36.96%),总有效率69.57%,差异有统计学意义(P<0.05)。治疗组MAP、HR、RR较治疗前明显降低,Pa O2较治疗前明显上升,与对照组之间存在显著性差异。结论无创呼吸机辅助通气联合吗啡可效改善患者呼吸功能,增强临床治疗效果,改善患者低氧血症,缩短病程,值得进一步推广应用。

Objective To discuss the clinical curative effect of non-invasive ventilator assisted ventilation combined with morphine in treatment of acute left heart failure. Methods 92 cases of patients with acute left heart failure treated in our hospital from January 2013 to January 2015 were selected and randomly divided into two groups with 46 cases in each, the control group were given non-invasive ventilator assisted ventilation treatment, the experimental group were given non-invasive ventilator assisted ventilation combined with morphine treatment, the indicators of total effective rate, mean arterial pressure（MAP）, heart rate（HR）, respiratory rate（RR）and mean arterial pressure（Pa O2） of the two groups were compared. Results In the treatment group, 27 cases were effective（58.70%）, 15 cases were markedly effective（32.61%）,the total effective rate was 91.31%, in the control group, 15 cases were effective（32.61%）, 17 cases were markedly effective（36.96%）, the total effective rate was 69.57%, the differences were obvious（P〈0.05）, the MAP, HR and RR were obviously decreased compared with those before treatment in the treatment group, the Pa O2 was obviously improved compared with that before treatment in the treatment group, which were obviously different from those in the control group. Conclusion Non-invasive ventilator assisted ventilation combined with morphine can effectively improve the patient＇s respiratory function, enhance the clinical treatment effect, improve the patient＇s hyoxemia and shorten the course of disease, which is worthy of further promotion and application.
