甘精胰岛素联合阿卡波糖治疗老年糖尿病的临床效果分析

Clinical Effect Analysis of Insulin Glargine Combined with Acarbose in the Treatment of Senile Diabetes

目的:评价甘精胰岛素联合阿卡波糖治疗老年糖尿病的临床效果。方法:选取2014年1月-2015年1月于笔者所在医院糖尿病科收治的老年2型糖尿病患者150例,采用随机数字表法将患者分为观察组(75例)和对照组(75例)。所有患者治疗前即停用原有口服降糖药物,并根据患者具体情况制定食谱建议和运动方案,两组患者均给予相同的饮食及运动指导,同时给予阿卡波糖治疗,观察组在此基础上给予甘精胰岛素治疗,对照组则给予精蛋白生物合成人胰岛素注射液治疗。比较两组患者血糖达标时间、糖化血红蛋白、空腹血糖及日平均血糖水平。结果:观察组患者血糖达标时间、糖化血红蛋白、空腹血糖及日平均血糖水平等指标均显著优于对照组(P<0.05)。观察组的低血糖发生率为4.0%,显著低于对照组的13.3%(P<0.05)。观察组患者有2例发生胃肠道反应,对照组有15例发生胃肠道反应,两组比较差异有统计学意义(P<0.05)。结论:甘精胰岛素联合阿卡波糖治疗老年糖尿病患者可有效控制血糖,且操作简单,患者依从性好,安全性高,值得在临床推广。

Objective：To evaluate the clinical effect of Insulin Glargine combined with Acarbose in the treatment of senile diabetes.Method：150 elderly patients with type 2 diabetes in our hospital from January 2014 to January 2015 were selected,according to the random number table method,they were divided into the observation group（75 cases） and the control group（75 cases）.All patients deactivated original oral hypoglycemic drugs,and developed recipes and exercise program recommendations based on the specific circumstances of the patients before treatment,they were given the same diet and exercise guidance,and given Acarbose treatment.The observation group was given Insulin Glargine,the control group was treated with Isophane Protamine Biosynthetic Human Insulin Injection.The blood sugar up to the standard time,glycated hemoglobin,fasting blood glucose and average daily glucose levels of the two groups were compared.Result：The blood sugar up to the standard time,glycated hemoglobin,fasting blood glucose and average daily glucose levels in the observation group were significantly better than those in the control group（P〈0.05）.Hypoglycemia of observation group was 4.0%,which was significantly lower than 13.3% of the control group（P〈0.05）.There were 2 cases of gastrointestinal reactions occurred in the observation group and 15 cases of gastrointestinal reactions occurred in the control group,the difference was statistically significant（P〈0.05）.Conclusion：Insulin Glargine combined with Acarbose in the treatment of elderly patients with diabetes can effectively control blood sugar,and the operation is simple,patient compliance is good, with high safety,it is worthy of clinical promotion.
