右美托咪定联合瑞芬太尼在日间乳腺手术中的应用 被引量:1

摘要 目的:探索右美托咪定联合瑞芬太尼在局部浸润麻醉下日间乳腺手术中的应用。方法:90例ASAⅠ~Ⅱ级择期乳腺手术病例,随机分成3组,每组30例,D-R组为右美托咪定联合瑞芬太尼组,R组为瑞芬太尼组,D组为右美托咪定组,三组均选择静脉全麻复合局部浸润麻醉。入手术室后常规面罩吸氧(3L/min),行心电监护,观察并记录给药前(T0);经微泵注射给药10min(T1);局部浸润麻醉(T2);手术切皮时(T3);术中30min(T4);术中60min(T5);术毕(T6)及术后2h(T7)等时间点的平均动脉压(MAP)、心率(HR)及Ramsay镇静评分和疼痛视觉模拟评分(VAS评分);并记录呼吸抑制和不良事件发生情况;血管活性药物使用情况。结果:与R组和D组相比,D-R组患者不同时点的MAP、HR的指标变化更平稳;与D组和R组相比,D-R组患者的Ramsay评分和VAS评分结果更好,差异显著;D-R组不良事件发生情况少于R组和D组,SPO2降低不良事件发生率明显少于R组,差异显著。结论:在日间乳腺手术中应用右美托咪定复合瑞芬太尼的镇静镇痛效果安全可靠。
