摘要 目的研究胎儿超声心动图技术对胎儿心脏结构及血流动力学的评估价值。方法整群选择2014年1—12月来该院就诊的342例胎儿作为研究对象,对所有研究对象行超声心动图检查,观察胎儿心脏结构及血流动力学状况,并与出生或引产结果比较,评价超声心动图的应用价值。结果所有胎儿中超声心动图检出225例(65.79%)存在心脏结构异常,灵敏度为99.11%,特异度为96.75%。正常胎儿二尖瓣血流E峰(MVE)、三尖瓣血流E峰(TVE)及三尖瓣血流A峰(TVA)、MVE/A及TVE/A显著低于心脏结构异常的胎儿,差异具有统计学意义(P<0.05)。两组二尖瓣血流A峰(MVA)比较,差异无统计学意义(P>0.05)。结论超声心动图对于诊断胎儿心脏结构异常具有较高临床价值,血流动力学指标可作为临床诊断先天性心脏病重要参考指标。 Objective To research the evaluation value of fetal echocardiography technique for fetal heart structure and hemodynamics. Methods 342 cases of fetuses treated in our hospital from January 2014 to December 2014 were selected as the research object and were given echocardiography examination, the fetal heart structure and hemodynamics were observed and compared with the born and induced labor results, the application value of echocardiography was evaluated. Results225 cases(65.79%)were with abnormal heart structure detected by echocardiography of all fetuses, the sensitivity was99.11%, the specificity was 96.75%, the mitral valve flow E peak(MVE), tricuspid valve flow E peak(TVE), tricuspid valve flow A peak(TVA), MVE/A and TVE/A of the normal fetuses were obviously lower than those of the fetuses with abnormal heart structure, and the differences were statistically significant(P〈0.05), the difference in the mitral valve flow A peak(MVA)between the two groups was not statistically significant(P〉0.05). Conclusion Echocardiography diagnosis of fetal abnormal heart structure has a higher clinical value, the hemodynamics indicators can be used as the important reference indicators in diagnosis of congenital heart disease.
