丙泊酚复合氟比洛芬脂在无痛人流手术中的应用

Application of Propofol Combined with Flurbiprofen Axetil in Painless Induced Abortion

目的研究丙泊酚复合氟比洛芬脂在无痛人流手术中的临床效果及安全性。方法整群选择该院2014年6月—2015年3月无痛人工流产的早孕妇女100例,随机分为两组,每组50例。A组丙泊酚复合芬太尼组,静脉注射芬太尼1 ug/kg,5 min后静脉注射丙泊酚2 mg/kg;B组为丙泊酚复合氟比洛芬脂,静脉注射氟比洛芬脂1 mg/kg,5 min后静脉注射丙泊酚2 mg/kg。观察两组麻醉镇痛效果,血压心率变化,呼吸抑制,术后唤醒时间,术后宫缩等参数。结果两组静脉用药后,MAP、HR、Sp O_2均有所下降,组间比较A组Sp O_2下降比B组明显,差异有统计学意义(P﹤0.05)。手术中A组呼吸抑制明显多于B组(P﹤0.05),术后唤醒时间B组明显短于A组,术后宫缩痛控制情况B组优于A组。结论丙泊酚复合氟比洛芬脂用于无痛人流手术中,麻醉效果确切安全有效,呼吸抑制减少,术后宫缩痛明显缓解。

Objective To study the clinical effect and safety of propofol combined with flurbiprofen axetil in painless induced abortion. Methods 100 women with early pregnancy underwent painless induced abortion in our hospital from June2014 to March 2015 were selected and randomly divided into two groups, group A（propofol combined with fentanyl group）,and group B（propofol combined with flurbiprofen axetil group） with 50 cases in each. Group A were given intravenous administration of fentanyl 1ug/kg, then propofol 2mg/kg 5 min later. Group B were given intravenous administration of flurbiprofen axetil 1mg/kg, then propofol 2mg/kg 5 min later. The narcotic analgesic effect, changes in blood pressure and heart rate, respiratory depression, postoperative wake-up time, postoperative uterine contraction and other parameters of the two groups were observed. Results After the intravenous administration of anesthetic drugs, the MAP, HR, and Sp O_2 decreased in both groups, furthermore, the decrease was more significant in group A than that in group B, with statistically significant difference（P0.05）. The incidence of intraoperative depression was much higher in group A than that in group B（P〈0.05）.The time needed for postoperatively waking up was significantly shorter in group B than in group A. The status of control of postoperative uterine contraction was much better in group B than in group A. Conclusion Propofol combined with flurbiprofen axetil applied to the painless induced abortion is safe and effective with exact narcotic analgesic effect, which can reduce the respiratory depression and significantly relieve the pain due to postoperative uterine contraction.
