摘要 目的分析研究急性颅脑损伤患者血B型脑钠肽水平的变化情况,总结B型脑钠肽水平和颅脑损伤之后颅内压之间的关系。方法随机选取2014年3月—2015年3月该院收治的急性颅脑损伤患者资料90例实施回顾性分析,依照患者在入院的时候的GCS评分情况将90例患者分为轻度颅脑损伤组30例,中度颅脑损伤组30例,重度颅脑损伤组30例,另外选取同期30例健康人群作为对照组,在患者入院当天、治疗3~5 d以及治疗15 d检测其血浆样本B型脑钠肽水平,对结果实施统计学分析。结果与对照组患者比较,颅脑损伤患者的血B型脑钠肽水平显著增加,差异有统计学意义(P〈0.05);治疗15 d,重度颅脑损伤组患者血B型脑钠肽水平(200.3±47.6)pg/m L明显高于轻度颅脑损伤组(75.1±41.8)pg/m L和中度颅脑损伤组(102.1±35.1)pg/m L,差异有统计学意义(P〈0.05)。结论血B型脑钠肽水平能够作为辨别颅脑损伤严重程度和损伤之后颅内压升高的评价指标。 Objective To analyze and study the changes of blood b-type brain natriuretic peptide level in patients with acute craniocerebral injury and summarize the relationship between b-type brain natriuretic peptide level and intracranial pressure after traumatic brain injury. Methods A retrospective analysis was conducted on the data of 90 cases with acute craniocerebral injury admitted from March 2014 to March 2015. The 90 patients were divided into mild craniocerebral injury group, moderate craniocerebral injury group and severe craniocerebral injury group, with 30 cases in each in accordance with the GCS scores on the admission. And other 30 healthy people were selected as the controls during the same period. The b-type brain natriuretic peptide level in the plasma samples was measured in the patients on the day of admission, 3~5 d and 15 d of treatment. The results of measurement were analyzed statistically. Results Compared with the control group, the blood b-type brain natriuretic peptide levels increased significantly in craniocerebral injury groups(P〈0.05).On the 15 d of treatment, the blood b-type brain natriuretic peptide level was much higher in the severe craniocerebral injury group than that in the mild craniocerebral injury group[(200.3±47.6) pg/m L vs(75.1±41.8) pg/m L] or that in the moderate craniocerebral injury group[(200.3±47.6) pg/m L vs(102.1±35.1) pg/m L] with statistically significant difference(P〈0.05).Conclusion Blood b-type brain natriuretic peptide level can be an index for evaluating the severity of craniocerebral injury and increased intracranial pressure after severe craniocerebral injury.
