摘要
目的通过检测HE4在卵巢上皮性恶性肿瘤、卵巢良性肿瘤、异位子宫内膜、正常内膜中的表达,把HE4作为一个肿瘤标记物,来鉴别盆腔肿物性质提供诊断依据。方法整群选取内蒙古人民医院2012年7月—2014年7月收治的167例手术患者,通过病理确诊:其中子宫内膜异位症的60例,卵巢良性肿瘤组织标本30例,正常内膜组织的因子宫病变而进行子宫全切术的患者30例,上皮性卵巢癌47例。采用免疫组化实验方法对各组石蜡切片进行化学染色,并进行对各组标本染色结果的染色强度进行评价,同时应用m Igae-or-plus对染色切片中的阳性部分测量其平均灰度值(mean IOD),并分析HE4在各组中的表达情况。结果子宫内膜异位症、卵巢上皮性恶性肿瘤、正常内膜中均有HE4的表达,HE4在卵巢良性肿瘤中的表达呈弱阳性或者阴性表达。HE4的表达在卵巢上皮性恶性肿瘤、子宫异位内膜、正常内膜、卵巢良性肿瘤中表达呈逐级下降趋势,HE4在四种不同组织中的表达的mean IOD值差异有统计学意义(P<0.05),而且进行两两组间比较均差异有统计学意义(P<0.05)。结论 1HE4在卵巢上皮性恶性肿瘤、子宫内膜异位症、正常内膜、卵巢良性肿瘤中的表达呈现逐级下降趋势,通过探索性研究,HE4可能作为一个良好的卵巢肿瘤标记物,有助于来鉴别盆腔肿物性质提供诊断依据;也表明HE4对肿瘤的发生、侵袭、转移、预后等发挥着重要作用。
Objective To detect the expression of HE4, a tumor marker, in patients with malignant epithelial ovarian cancer, benign ovarian tumor, endometriosis and normal endometrium so as to provide a diagnostic base for the identification of pelvic tumor. Methods In this study, 167 tissue specimens are from the Department of Pathology, Inner Mongolia People＇s Hospital, from July 2012 to July 2014. Totally 60 endometriosis samples were investigated as the experimental group, the benign ovarian tumor tissue samples from 30 cases and normal endometrium tissue samples from 30 cases underwent hysterectomy due to uterine lesions were set as the negative control respectively, and 47 samples of epithelial ovarian cancer as the positive control group. The expression of HE4 was examined by immunohistochemical staining between experimental group and control group. The expression intensity of HE4 was estimated by the microscope. The mean IOD of positive expression rates of HE4 were determined by m Igae-or-plus. The expression of HE4 in the groups was analyzed. Results Endometriosis, malignant epithelial ovarian tumor and normal endometrium had the distinct expression of HE4. The expression of HE4 was weakly positive or negative in benign ovarian tumor. The expression of HE4 showed a step-down trend in the malignant epithelial ovarian tumor, endometriosis, normal endometrium and benign ovarian tumor, and the mean IOD value of HE4 expression in the four different tissues had significant difference（P〈0.05）, and pairwise comparisons were statistically significant（P〈0.05）. Conclusion The expression of HE4 showed a step-down trend in the malignant epithelial ovarian tumor, endometriosis, normal endometrium and benign ovarian tumor. The study indicated that HE4 may be a good epithelial ovarian tumor marker, which provides diagnostic basis for identifying the nature of pelvic tumor; and plays an important role in the oncogenesis, invasion, metastasis, and outcome of the tumor.
