左旋氨氯地平对糖尿病并高血压患者早期肾损害的影响

Effect of Levamlodipine Besylate on Early Renal Damage in Patients with Diabetes and Hypertension

目的探讨左旋氨氯地平对糖尿病并高血压患者早期肾损害的影响。方法选择符合2型糖尿病高血压早期肾损害的患者100例患者,随机平均分为实验组和对照组。实验组患者给予苯磺酸左旋氨氯地平,对照组予以维生素C治疗,治疗时间为8周。观察治疗前后血压控制情况;肾损害指标:尿微量白蛋白(Microalbumin MA),α1-微球蛋白(α1-microglobulin,α1-MG),β2-微球蛋白(β2-microglobulin,β2-MG)和胱抑素C(cystatin C,Cys C);检测患者肾动脉血流参数。结果治疗组24 h平均收缩压,24 h平均舒张压均较治疗前下降(P<0.05);MA,α1-MG,β2-MG和Cys C等指标,治疗后明显好于治疗前,差异有统计学意义(P<0.05);治疗后治疗组肾动脉收缩期峰值流速(Vs)、舒张末期流速(Vd)均较前有所提高,肾动脉血流阻力指数RI下降,差异有统计学意义(P<0.05)。结论左旋氨氯地平可以有效地降低24 h血压,并且对早期肾功能损害起到很好的保护作用。

OBJECTIVE To observe the effect of Levamlodipine Besylate on early renal damage in patients with diabetes and hypertension. METHODS A total of 100 patients with type 2 diabetes and hypertension were randomly divided into experimental group and control group. The patients were treated with Levamlodipine Besylate in the experimental group and vitamin C in the control group for 8 weeks. The observed indexes were blood pressure control,renal damage index： microalbumin（ MA）,α1-microglobulin（ α1-MG）,β2-microglobulin（ β2-MG） and serum cystatin C（ Cys C）,the parameters of renal artery blood flow. RESULTS In patients of the experimental group,the levels of 24 h average systolic blood pressure,24 h average diastolic blood pressure were decreased than those before the treatment,the difference was statistically significant（ P 〈0.05）; the indexes of MA,α1-MG,β2-MG and Cys C were better than those before the treatment,the difference was statistically significant（ P〈0.05）; the renal artery peak systolic velocity（ Vs）and diastolic velocity（ Vd） were increase,renal artery blood flow resistance index（ Ri） were decreased than those before treatment,the difference was statistically significant（ P〈 0. 05）. CONCLUSION Levamlodipine Besylate can reduce effectively the blood pressure of 24 h,and has a good protective effect on early renal damage.
