308nm准分子光联合C02点阵激光治疗白癜风的临床观察

The Clinical Observation of 308 nm Excimer Laser Light Therapy Combined with C02 Laser Therapy of Vitiligo

目的观察308 nm准分子光联合CO2点阵激光治疗白癜风的疗效和安全性。方法针对35例白癜风患者76处皮损,将每一处白斑处皮损沿长轴分为两侧,一侧为治疗组,先进行点阵激光治疗,20～30 min后将患处白斑皮损照射308 nm准分子光;未行点阵激光的另一侧为对照组。308 nm准分子光每周照射2～3次,C02点阵激光每2周1次,连续治疗观察16周,治疗结束3 d后评价疗效。结果治疗组与对照组总有效率分别为90.3%和71.1%。治疗组与对照组显效率在面颈部分别为81.2%和62.6%,躯干部分别为69.9%和40.8%,四肢部位分别为62.0%和35.0%。治疗组与对照组治疗次数以308 nm准分子光治疗次数计算比较：面颈部分别为（7.12±1.12）次和（10.41±1.98）次,躯干部分别为（15.20±2.94）次和（18.42±3.22）次,四肢部位分别为（17.19±3.41）次和（21.12±4.46）次,所有患者在治疗过程中均未出现严重不良反应。结论 308 nm准分子光联合CO2点阵激光治疗白癜风有效而且安全,疗效优于单用308准分子光。

Objective To observe 308- nm excimer joint lattice C02 laser light efficacy and safety for the treatment of vitiligo. Methods 35 cases of patients with vitiligo 76 lesions,each place white spot lesions along the long axis was divided into a two parts. Half of the side were the treatment group,firstly treated with fractional laser,20- 30 min later leukoplakia lesions simultaneously irradiating the affected area after the 308 nm excimer light; the other side did not undergo fractional laser as the control group. The frequency of308- nm excimer light irradiation was 2- 3 times a week,C02 laser and lattice laser was once every 2 weeks,continuous observation 16- weeks treatment,curative effect evaluation after 3d. Results The treatment group and control group total effective rate were 90. 3% and 71. 1% respectively. The total effciency rate of the treatment group and control group in the face and neck were 81. 2% and 62. 6% respectively,hindquarters cadres were 69. 9% and 40. 8% respectively,limb parts were 62. 0% and 35. 0% respectively. Treatment group and control group（ treated with 308 nm excimer light frequency calculation comparison： face and neck,respectively（ 7. 12 ± 1. 12） and（ 10. 41 ± 1.98） times,hindquarters cadres,respectively（ 15. 20 ± 2. 94） and（ 18. 42 ± 3. 22） times,limb parts respectively（ 17. 19 ± 3. 41） and（ 21. 12 ± 4. 46） times,all of the patients in the course of treatment had no severe adverse reaction occurred. Conclusion 308 nm excimer light C02 fractional laser treatment of vitiligo joint could be effective and safe,and more effective than single- use 308 excimer light.
