无创通气联合振动排痰治疗AECOPD疗效评价

Evaluation on affection of noninvasive ventilation combined with vibration sputum on patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)

目的观察无创通气联合振动排痰技术对AECOPD患者的作用,评价其临床应用价值。方法选择住院的AECOPD患者75例,随机分为治疗组、阳性对照及阴性对照组,治疗前及治疗后1周分别监测患者动脉血p H、PaO_2、PaCO_2、SaO_2,静脉血PCT,肺功能FEV_1、FEV_1/FVC、DLCO的变化;采用SPSS13.0软件分析数据。结果无创通气联合振动排痰技术治疗AECOPD患者动脉血气PaO_2、PaCO_2均较阳性对照及阴性对照组有显著性差异(P<0.01),SaO_2、静脉血PCT较阳性对照及阴性对照组有明显差异(P<0.05),肺功能FEV_1、FEV_1/FVC、DLCO较阳性对照及阴性对照组有显著性差异(P<0.01)。结论应用无创通气联合振动排痰技术治疗AECOPD患者,可更好改善通气及氧合功能,减轻炎症反应,通畅气道,提高无创通气技术应用成功率。

Objective To investigate the affection of noninvasive ventilation combined with vibration sputum on patients with AECOPD. Methods Seventy-five patients with AECOPD in hospital were selected,and randomly divided into the treatment group,the positive contrast group and the negative contrast group. Before and after treatment for one week,the PH,PaO2,PaCO2,SaO2 in arterial blood,Procalcitonin（ PCT） in Venous blood,FEV1,FEV1/ FVC,DLCO were measured. SPSS 13. 0 software was used to analyze the data. Results After treatment,the PaO2、PaCO2 among the treatment group and the negative contrast group and the positive contrast group had a significant differences（ P〈 0. 01 / P〈 0. 05）,while the Sa02 and PCT in these groups had a markedly differences（ P〈 0. 05）; FEVl,FEVl / FVC,DLCO between the treatment group and the negative contrast group and the positive contrast group had a significant differences（ P〈 0. 01 / P〈 0. 05）. Conclusions Application of noninvasive ventilation combined with vibration sputum in the treatment of patients with AECOPD could better improve the ventilation and oxygenation index, reduce inflammation, airway and improve the successful application of noninvasive ventilation technical.
