穴位埋线治疗稳定期慢性阻塞性肺病疗效的临床多中心研究

Clinical Evaluation on Efficacy of Chronic Obstructive Pulmonary Disease with Acupoint Catgut Embedding: A Multi- central Randomized Controlled Trial

目的:对穴位埋线治疗慢性阻塞性肺病(COPD)稳定期的疗效进行多中心临床评价。方法:采用平行对照、随机分组、多中心试验的方法进行临床研究,将143例患者随机分为埋线组和对照组,以中医症状评分、六分钟步行试验(6MWT)、改良英国MRC呼吸困难指数(m MRC)、COPD评估测试标准(CAT)及医院焦虑抑郁情绪测量表(HAD)为观察指标进行比较,两组均在治疗3个月后进行评分,并进行统计学处理、分析比较。结果:最终完成试验病例143例,两组患者经治疗后,中医症状评分、CAT评分均有明显降低,且有统计学意义(P<0.01),且埋线组改善的程度大于对照组(P<0.01);两组治疗后的6MWT均有上升(P<0.01),且有统计学意义(P<0.01),埋线组上升的趋势大于对照组(P<0.01)。两组患者的焦虑评分治疗前后均有所下降(P<0.05),且埋线组的焦虑评分低于对照组(P<0.05);两组患者的抑郁评分在治疗前后均有所下降,但无统计学意义(P>0.05)。结论:微创埋线联合西药治疗慢性阻塞性肺病稳定期患者可改善其临床症状,提高活动耐量及改善焦虑情绪,且安全可靠。

Objective： To evaluate the efficacy of acupoint catgut embedding on treating chronic obstructive pulmonary disease. Methods： 143 cases were randomly divided into a catgut embedding therapy group and a control group randomly,with multicenter trial method in clinical study. TCM symptom score and six minutes walking distance（ 6MWT）,the improved MRC dyspnea index（ m MRC）,COPD assessment test standard（ CAT）,anxiety and depression emotion measurement table as observation indexes were compared in two groups after treatment and in 3 months with statistical analysis. Results： 143 cases were finished in the end of the experiment. The treatment group had 7 cases and the control group had 8 cases. After treatment,clinical symptoms score,CAT score were decreased significantly in both groups（ P 〈0. 01）,and the degree of improvement in the catgut embedding group was improved significantly than that in the control group（ P 〈0. 01）, After treatment,the 6MWT in two groups had significantly increased than before（ P 〈0. 01）,and the catgut embedding group increased more than the control group（ P 〈0. 01）. Anxiety scores on two groups were decreased after treatment（ P 〈0. 05）,and anxiety score of catgut embedding group was lower than that of the control group（ P 〈0. 05）, in two groups the depression scores decreased after treatment,and there was no statistical significance（ P 〉0. 05）.Conclusion： Catgut embedding therapy combined with western medicine treatment can improve the clinical symptoms of patients with chronic obstructive pulmonary disease in stable phase,and can improve activity tolerance and improve the anxiety,which is safe and reliable.
