脑卒中后血管性认知障碍影响因素的Meta分析

Meta analysis of influencing factors of vascular cognitive impairment after stroke

[目的]评价脑卒中后血管性认知障碍的影响因素.[方法]应用Meta分析对脑卒中后血管性认知障碍影响因素的研究结果进行定量综合分析,运用Revman5.0进行异质性检验及计算合并OR 值、95%置信区间（95%CI）.[结果]检索到符合纳入标准文献10篇,共纳入脑卒中病人3343例,脑卒中后伴认知障碍病人1221例,脑卒中后不伴有认知障碍病人2122例.Meta分析结果显示,高血压、高脂血症、糖尿病、冠心病、吸烟的OR 值分别为3.34,2.68,1.85,1.86,3.80,95%CI 分别为（2.35-4.76）,（2.26-3.19）,（1.51-2.26）,（1.53-2.26）,（1.75-8.23）,合并稳定性较好;饮酒者OR 值为1.19,95%CI 为0.55-2.59.[结论]高血压、高脂血症、糖尿病、冠心病、吸烟是脑卒中认知障碍的影响因素,尚不能确定饮酒与脑卒中后认知障碍的关系.护理人员在护理脑卒中病人时,应重视脑卒中认知障碍的影响因素.

Objective:To evaluate influencing factors of vascular cognitive impairment after stroke.Methods:Meta analysis was used to analyze the influencing factors of vascular cognitive impairment after stroke,Revman 5.0was used to do Heterogeneity test,ORvalue and 95%confidence interval(95%CI).Results:A total of 3 343 cases of stroke patients,1 221 patients with cognitive impairment after stroke and 2 122 patients without cognitive impairment after stroke were included in the 10 articles.Meta analysis results showed that the ORvalues of hypertension,hyperlipidemia,diabetes,coronary heart disease,smoking were respectively 3.34,2.68,1.85,1.86,3.80;95%CI were respectively(2.35-4.76),(2.26-3.19),(1.51-2.26),(1.53-2.26),(1.75-8.23)and stability was better;ORvalue of drinkers was 1.19,95%CI was 0.55 to 2.59.Conclusion:Hypertension,hyperlipidemia,diabetes mellitus,coronary heart disease and smoking were the influencing factors of cognitive impairment in stroke patients,and the relationship between drinking and cognitive impairment after stroke was not determined.Nursing personnel should pay attention to the influencing factors of cognitive impairment in patients with stroke.
