摘要
目的探讨3项炎性指标联合检测在鉴别诊断新生儿感染中的临床效果及应用价值,为早期诊断新生儿感染、及早治疗提供参考依据。方法将医院于2014年2月-2015年7月收治的113例感染新生儿作为研究对象,按照感染类型的不同分为细菌感染组62例和病毒感染组51例,同期选择50名健康新生儿作为对照组;对比3组新生儿血降钙素原(PCT)、白细胞(WBC)及C-反应蛋白(CRP)水平,数据采用SPSS 19.0软件进行统计分析。结果治疗前细菌感染组、病毒感染组新生儿PCT、CRP及WBC水平明显高于对照组;治疗后细菌感染组和病毒感染组新生儿PCT、CRP及WBC水平均明显下降,与治疗前比较差异有统计学意义(P<0.05);3种联合检测方法与PCT单项检测比较,差异有统计学意义(P<0.05)。结论在鉴别诊断新生儿感染,PCT、WBC及CRP水平均可以作为有效指标,而相对于其他联合检测,PCT、CRP及WBC 3项指标联合检测的诊断准确率更高,在鉴别诊断及疗效评价上具有较高的应用价值,值得在临床推广。
OBJECTIVE To investigate clinical effect and application value of procalcitonin(PCT),white blood cell(WBC)and C reactive protein(CRP)in differential diagnosis of neonatal infection,to provide reference for early diagnosis and early treatment of neonatal infection.METHODS From Feb.2014 to Jul.2015,113 cases of children with neonatal infections in our hospital were selected as the research subjects.According to different types of infections,those children were divided into the bacterial infection group(62cases)and viral infection group(51cases).At the same time,50 healthy newborns were selected as control group.Levels of CRP,WBC and PCT among the three groups were compared.Data were analyzed with SPSS 19.0.RESULTS Before treatment,levels of WBC,CRP and PCT in bacterial infection group and virus infection group were significantly higher than those in control group.After treatment,levels of WBC,CRP and PCT in bacterial infection group and viral infection group significantly decreased,and the difference was significant(P〈0.05).The results of combined detection of three indicators and single PCT detection were compared;the difference was significant(P〈0.05).CONCLUSIONIn the diagnosis of neonatal infections,levels of PCT,WBC and CRP can be used as effective indicators.Compared with the other joint detection,the diagnostic accuracy of three indicators of PCT,CRP and WBC was higher.It has very high application value in diagnosis and efficacy.It is worthwhile to be popularized clinically.
