摘要
目的探讨FHIT基因在90例肝癌手术标本中表达差异与临床病理因素的相关性。方法收集2010年1月至2013年9月中90例临床资料和随访记录完整的原发性肝癌患者的手术标本。以相应的癌旁组织为对照,以免疫组化染色技术制作组织芯片,观测上述标本中FHIT基因表达差异,分析FHIT基因与肝癌临床病理因素和术后生存期的关系及意义。结果(1)FHIT基因表达水平高低与肝癌细胞分化程度正相关(CC=0.589,P〈0.05),并与淋巴结转移有正相关性(CC=0.326,P〈0.05);(2)肝癌手术患者的生存期与FHIT基因蛋白的低表达、肝癌细胞分化、肝癌的病理分级、TNM分期和肿瘤大小密切相关,差异有统计学意义(P〈0.05),但与性别、年龄、有无包膜、AFP水平、有无门静脉癌栓、淋巴结转移和合并肝硬化无明显相关性(P〉0.05)。结论FHIT基因是一种抑癌基因,其在肝癌细胞中的表达水平差异与肝癌细胞的分化程度、淋巴结转移密切正相关,FHIT基因参与了肝癌的发生发展过程,并由此与肝癌手术后生存期的长短正相关。
Objective To explore the expression differences of the FHIT gene in human hepatocellular carcinoma tissues and para-carcinoma tissues, and to analyze the relationship between FHIT gene expression and clinicopathological features of liver cancer. Methods In 90 cases of primary liver cancer, the clinic pathological data and follow-up records were collected, surgical specimens were kept in paraffin blocks, FHIT gene expression was determined by IHC-tissue chip. The relationship between the FHIT gene and the clinicopathological factors and postoperation survival were analyzed. Results (1) FHIT protein expression levels were correlated with tumor differentiation ( CC = 0. 589, P 〈 0. 05 ) and lymph node metastasis ( CC = 0. 326, P 〈 0.05 ). (2) The overall postoperative survival was correlated with low expression level of FHIT protein, tissue differentiation of liver cancer, clinicopathological T stage, TNM stage and tumor size (P 〈 0. 05), but uncorrelated with gender, age, existence of tumor capsule, portal vein tumor emboli, AFP level, existence of cirrhosis and lymph node metastasis (P 〉 0.05 ) . Conclusions FHIT gene is a cancer suppressor gene correlated with tissue differentiation of liver cancer and lymph node metastasis. FHIT gene may be involved in tumor invasion and progression correlates with postoperative overall survival. Poor expression of FHIT gene on hepatoma cells indicates increased tumor malignancy.
