右美托咪啶复合达克罗宁胶浆表麻用于纤支镜引导清醒气管插管的临床观察被引量：7

摘要目的观察右美托咪啶复合达克罗宁胶浆表麻用于纤支镜引导清醒气管插管的安全性和有效性。方法将评估为困难气道的80例患者随机分为两组,先给予表麻,再静脉泵注右美托咪啶。A组用利多卡因表麻,B组用达克罗宁胶浆表麻。观察并比较两组患者入室时（T0）、表麻5min后（T1）、右美托咪啶给药20min后（T2）、纤支镜置入时（T3）、插管时（T4）、插管完成后1min（T5）、拔管时（T6）的HR、MAP、Sp02和RR,记录患者插管时气管插管评分,拔管时Ramsay镇静评分,术后随访咽痛和声音嘶哑例数,患者满意度。结果两组患者均顺利完成插管,与A组比较,B组T2-T6时HR减慢（P〈0.05）;MAP降低（P〈0.05）;RR减慢（P〈0.05）,但两组中SpO2的变差异不大（P〉0.05）。B组插管时气管插管评分、拔管时Ramsay镇静评分优于A组（P〈0.05）。术后随访B组咽痛和声音嘶哑的发生率低于A组（P〈0.05）。患者满意度于A组（P〈0.05）结论使用右美托咪啶复合达克罗宁胶浆表麻用于纤支镜引导清醒气管插管能有效降低患者的应激反应,维持血流动力学平稳,降低术后咽痛和声音嘶哑的发生率。
