自拟活血健脾解毒方治疗冠心病并发急性肾衰竭的临床观察

Clinical Observation of Huoxue Jianpi Jiedu Decoction on Coronary Heart Disease Complicated with Acute Renal Failure

目的观察自拟活血健脾解毒方治疗冠心病并发急性肾衰竭的疗效及对中医证候积分和心肾功能的影响。方法 120例患者按随机数字表法分为对照组和治疗组。对照组给予西医常规治疗,治疗组在对照组的基础上给予活血健脾解毒方治疗,1个月后比较两组患者的临床疗效和中医证候积分,心脏彩超检测左室舒张末径(LVDEd)、左室收缩末径(LVSEd)、左房内径(LAd)等心功能指标,全自动生化分析仪检测肾功能指标尿素氮(BUN)、血肌酐(Cr)、内生肌酐清除率(Ccr)肾功能指标。结果治疗组总有效率为95.00%,优于对照组的83.00%(P<0.05)。治疗前两组倦怠乏力、腰膝酸软、畏寒肢冷、心悸、恶心呕吐及中医证候总积分比较,差别不大(P>0.05)。治疗后,两组倦怠乏力、腰膝酸软、畏寒肢冷、心悸、恶心呕吐及中医证候总积分均低于治疗前(均P<0.05),且治疗组中医证候积分低于对照组(均P<0.05)。治疗前两组LVDEd、LVSEd、LAd比较,差别不大(P>0.05)。治疗后两组LVDEd、LVSEd、LAd均低于治疗前,且治疗组显著低于对照组(均P<0.05)。两组治疗前BUN、Cr及Ccr比较,差别不大(P>0.05)。治疗后两组BUN、Cr及Ccr与治疗前比较均改善(P<0.05),且治疗组BUN和Cr下降,Ccr升高均优于对照组(P<0.05)。结论活血健脾解毒方治疗冠心病并发急性肾衰竭可提高治疗有效率,降低中医证候积分,改善心肾功能,临床疗效确切。

Objective：To observe the clinical effects of Huoxue jianpi jiedu decoction on coronary heart disease complicated with acute renal failure.Methods：120 patients were divided into the control group and the treatment group with random number table method.The control group were given conventional western medicine treatment,while the treatment group were added Huoxue jianpi jiedu decoction.The clinical curative effect and integral of TCM syndrome,LVDEd,LVSEd,Lad,BUN,Cr,Ccr were compared after 1 month.Results：The total effective rate was 95.00% in the treatment group,better than that of control group 83.00%（P〈0.05）.There was no difference in the languid,lumbar debility,aversion to cold and cold limbs,heart palpitations,nausea and vomiting and total integral of TCM syndrome between two groups before treatment（P〉0.05）.After treatment,all the data above were lower than those before treatment（P〈0.05）,and the scores of traditional Chinese medicine in the treatment group was lower than that of the control group（P〈0.05）.There was no difference in the level of LVDEd,LVSEd,Lad,BUN,Cr and Ccr between two groups before treatment（P〉0.05）.After treatment,levels of LVDEd,LVSEd,LAd were lower than those before treatment,and the treatment group was significantly lower than the control group（P〈0.05）.The levels of BUN,Cr and Ccr were improved compared with those before treatment,and the treatment group was superior to the control group（P〈0.05）.Conclusion：Huoxue jianpi jiedu decoction on coronary heart disease complicated with acute renal failure can improve the treatment efficiency,reduce the integral of TCM syndrome,improve heart function and enhance the clinical curative effect.
