婴儿脑积水分流术后硬膜下积液的临床分析 被引量:1

摘要 目的探讨婴儿脑积水脑室-腹腔分流术(VPS)后硬膜下积液的临床特征。方法回顾性分析2011年6月至2014年10月收治的19例婴儿脑积水VPS术后硬膜下积液的临床资料。结果 VPS至出现硬膜下积液的时间为1~128 d,中位时间为7d,其中VPS后1周内发生11例(57.9%),1月内16例(84.2%)。18例非血肿型硬膜下积液随访中,积液演变成慢性硬膜下血肿2例;16例(88.9%,16/18)积液自然吸收,另外2例积液体积明显减少。1例血肿型硬膜下积液经钻孔硬膜下血肿外引流术治愈。结论婴儿脑积水VPS后硬膜下积液多在分流术后1月内发生,非血肿型硬膜下积液几乎不引起临床占位效应,且能够自然吸收,通常不需要处理;血肿型硬膜下积液因易导致临床占位效应,一般需要采取外科干预措施。
