窒息新生儿尿微量白蛋白、αl-微球蛋白和转铁蛋白检测对早期肾功能损伤的诊断价值 被引量:7

摘要 目的探讨窒息新生儿尿微量白蛋白(MA)、αl-微球蛋白(αlM)和转铁蛋白(TRU)在早期肾功能损伤中的诊断价值。方法选取我院110例窒息新生儿作为研究组,根据窒息严重程度分为轻度窒息组(67例)和重度窒息组(43例),选取同期我院出生非窒息新生儿50例作为对照组,分别检测并比较各组新生儿MA、αlM、TRU及尿素氮(BUN)、血肌酐(Cr)水平。结果研究组新生儿MA、αlM、TRU、Cr水平均高于对照组,P<0.05;两组新生儿BUN水平比较差异不显著,P>0.05。重度窒息组新生儿MA、αlM、TRU、Cr水平均高于轻度窒息组,P<0.05;两组新生儿BUN水平比较差异不显著,P>0.05。结论窒息新生儿MA、αlM、TRU水平可作为诊断早期肾功能损伤的临床检测指标,并可反映肾损伤的严重程度。 Objective To investigate the diagnostic value of microalbuminuria (MA), α1-microglobulin (α1M) and transferrin (TRU) in early renal damage of asphyxial newborns. Methods 110 asphyxial newborns treated in our hospital were set as study group and divided into mild asphyxia group (67 cases) and severe asphyxia group (43 cases) according to the severity of asphyxia. 50 newborns without asphyxia in our hospital at same period were set as control group. The levels of MA, α1M, TRU, blood urea nitrogen (BUN), serum creatinine (Cr) were detected and compared. Results The levels of MA, α1M, TRU, Cr of study group were significantly higher than those of control group, P 〈0.05; the level of BUN of two groups had no statistical difference, P 〉0.05. The levels of MA, α1M, TRU, Cr of severe asphyxia group were significantly higher than those of mild asphyxia group, P 〈0.05; the level of BUN of two groups had no statistical difference, P〉0.05. Conclusions The levels of MA, α1M, TRU can be used as clinical detection indices in diagnosis of early renal damage of asphyxial newborns, and also can reflect the severity of renal injury.
