Application Research of GLISSON Pedicle Transverse Precise Hepatectomy in Clinic

目的研究GLISSON蒂横断式精准肝切除在临床中的应用效果。方法选取我院2010年6月至2015年6月收治的60例肝癌患者,并随机分为两组。对照组(28例)实施Pringle式肝癌切除术,观察组(32例)实施GLISSON蒂横断式精准肝切除术。比较两组手术情况、肝功能及并发症发生率。结果观察组手术时间明显长于对照组,术后第7 d肝功能恢复情况优于对照组,差异具有统计学意义(P<0.05);观察组术中出血量及阻断时间与对照组比较无统计学差异(P>0.05)。观察组并发症发生率为15.6%,明显低于对照组的39.3%,差异具有统计学意义(P<0.05)。结论 GLISSON蒂横断式精准肝切除手术治疗肝癌患者,操作简单,定位精确,术后并发症较少,肝功能损伤减轻,值得临床推广应用。

Objective To study the application effect of GLISSON pedicle transverse precise hepatectomy in clinic. Methods 60 patients with liver cancer admitted to our hospital fi＇om June 2010 to June 2015 were randomly divided into two groups. The control group （28 cases） was implemented Pringle hepatectomy, the observation group （32 cases） was implemented GLISSON pedicle transverse precise hepatectomy. The operation conditions, liver function and complications of two groups were compared. Results The operation time of observation group was longer than that of control group, the recovery of liver function 7 d after operation in observation group was better than that in control group, the difference was statistical （P〈0.05）; the intranperative bleeding volume and blocking time in two groups had no statistical difference （P〉0.05）. The incidence of complications of observation group was 15.6%, lower than 39.3% of control group, with statistical difference （P 〈0.05）. Conclusions For patients with liver cancer, GLISSON pedicle transverse precise hepatectomy has simple operation, accurate location, low complications and slight liver damage, which is worthy of promotion and application.
