地中海贫血产妇妊娠期血红蛋白、铁蛋白水平与妊娠结局分析

Analysis on the levels of Hemoglobin and Ferritin and Pregnancy Outcomes During Pregnancy in Pregnant Women with Thalassemia

目的分析轻型地中海贫血产妇妊娠期血红蛋白(HGB)、铁蛋白水平(SF)及妊娠结局。方法选取2013年10月至2015年10月我院妇产科收治的48例轻型地中海贫血产妇作为研究组,另选取同期健康孕妇48例作为对照组。比较两组的妊娠结局以及不同孕周的HGB、SF水平。结果研究组孕早期、孕中期、孕晚期的HGB和SF水平均显著低于对照组,且剖宫产、羊水过少/多、产后出血、早产儿、足月低体重儿发生率均显著高于对照组,差异均具有统计学意义(P<0.05)。结论轻型地中海贫血孕妇的HGB和SF水平随着孕期的增加而显著降低,早产儿、剖腹产率高于正常孕妇,新生儿体重偏低,临床需注意加强孕妇保健工作。

Objective To analyze the hemoglobin （HGB）, serum ferritin （SF） levels and pregnancy outcomes in pregnant women with mild thalassemia. Methods 48 cases of pregnant women with mild thalassemia treated in the department of gynecology and obstetrics of our hospital from October 2013 to October 2015 were selected as study group, and 48 cases of healthy pregnant women were selected as control group. The pregnancy outcomes and HGB and SF levels at different gestational weeks were compared between two groups. Results The levels of HGB and SF in study group at the first-trimester, second-trimester and third-trimester were significantly lower than those in control group, and the incidence of cesarean section, oligohydramnios/polyhydramnios, postpartum hemorrhage, premature infants and full-term low birth weight infants in study group were significantly higher than those in control group, all the difference was statistical （P 〈0105）. Conclusions In pregnant women with mild thalassemia, the levels of HGB and SF decrease significantly with gestation weeks, and the incidence of cesarean section and premature infants is higher than that in healthy pregnant women, and the birth weight of infants is low. The maternal health works should be strengthened in clinic.
