不同部位重型颅脑损伤患者内皮素-1的变化及临床意义分析 被引量:1

摘要 目的探讨不同部位重型颅脑损伤患者内皮素-1(ET-1)变化及临床意义。方法选取2014年2月至2015年10月我院神经内科收治的重型颅脑损伤患者70例作为研究组,选取同期我院神经内科收治的非颅脑损伤患者50例作为对照组。同时根据颅脑损伤部位将研究组分为脑实质损伤组(33例)和脑室脑池蛛网膜下腔出血组(37例)。比较各组间ET-1、空腹血糖(FBG)、C反应蛋白(CRP)水平。结果研究组患者ET-1、FBG及CRP水平均显著高于对照组,差异具有统计学意义(P<0.05)。脑实质损伤组患者ET-1水平显著低于脑室脑池蛛网膜下腔出血组,差异具有统计学意义(P<0.05);但两组FBG及CRP水平比较无显著差异(P>0.05)。结论重型颅脑损伤患者ET-1、FBG及CRP水平均明显升高,脑室脑池蛛网膜下腔出血患者ET-1水平显著升高,可作为临床诊断、判定预后的重要指标。 Objective To investigate the clinical significance and changes of endothelin-1 (ET-1) in patients with different parts of severe brain injury. Methods From February 2014 to October 2015, 70 patients with severe brain injury admitted to the neurosurgery department of our hospital were selected as study group, and 50 non-brain injury patients treated in our hospital at same period were set as control group. According to the brain injured parts, the study group was subdivided into brain parenchyma injury group (33 cases), ventricular and cisternal subarachnoid hemorrhage group (37 cases). The levels of ET-1, fasting blood glucose (FBG) and C-reactive protein (CRP) were compared. Results The levels ofET-1, FBG and CRP in study group were significantly higher than those in control group, with statistically significant difference (P 〈0.05). The level of ET-1 in brain parenchyma injury group was significantly lower than that of ventricular and cistemal subarachnoid hemorrhage group, with statistical difference (P〈0.05); but the levels of FBG and CRP in two groups had no statistical difference (P 〉0,05). Conclusions The levels of ET-1, FBG and CRP in patients with severe brain injury increase significantly, and the level of ET-1 in patients with ventricular and cisternal subarachnoid hemorrhage increases significantly. The results can be used as important indicators for clinical diagnosis and determination of prognosis.
