小剂量甲状腺素联合厄贝沙坦氢氯噻嗪治疗老年重症心力衰竭的观察 被引量:6

摘要 目的研究小剂量甲状腺素联合厄贝沙坦氢氯噻嗪治疗老年重症心力衰竭的临床疗效。方法选择2014年1月至2015年1月我院收治的80例老年重症心力衰竭患者为研究对象,应用随机数字表法分为常规治疗组和甲状腺组各40例。常规治疗组采用常规疗法辅以厄贝沙坦氢氯噻嗪治疗,甲状腺组在常规组治疗的基础上添加小剂量甲状腺素治疗,比较两组的临床疗效。结果治疗后,甲状腺素组的总有效率(92.50%)优于常规治疗组(67.50%),两组差异具有统计学意义(P<0.05);治疗后甲状腺组左室射血分数(LVEF)、B型钠尿肽(BNP)均优于常规治疗组,差异具有统计学意义(P<0.05)。结论在老年重症心力衰竭的常规治疗中,应用小剂量甲状腺素联合厄贝沙坦氢氯噻嗪进行治疗,临床疗效优越,可显著改善患者心功能指标,值得临床推广。 Objective To investigate the clinical effects of low dose L- thyroxin combined with Irbesartan and Hydrochlorothiazide in treating senile severe heart failure. Methods 80 cases with severe heart failure in our hospital from January 2014 to January2015 were selected and randomly divided into the conventional treatment group and the thyroid group,with 40 cases in each one. The conventional treatment group was given conventional treatment combined with Irbesartan and Hydrochlorothiazide. The thyroid group was given low dose of L- thyroxin based on the conventional treatment. The clinical effects of the two groups were compared. Results Total efficiency of thyroid group( 92. 50%) was higher than that in the conventional treatment group( 67. 50%),the difference was statistically significant( P〈0. 05). After treatment,LVEF and BNP of the thyroid group were better than those of the conventional treatment group,with the statistically significant difference( P〈0. 05). Conclusion In the treatment of patients with senile severe heart failure,low dose of L- thyroxin combined with Irbesartan and Hydrochlorothiazide has better clinical effects and can improve heart function parameters of the patients greatly,so it is worthy of clinic promotion.
