摘要
目的:探讨动态动脉硬化指数(AASI)对糖尿病患者靶器官损害的预测价值。方法:选取2012年1月-2014年1月本院收治入院的2型糖尿病患者100例,纳入观察组,根据AASI水平将2型糖尿病患者分为两组:AASI≤0.51为低水平组,AASI>0.51为高水平组。另选择本院同期健康体检者100例作为对照组,分别对AASI、尿白蛋白/尿肌酐比值(ACR)、颈动脉内-中膜厚度(IMT)进行检测,并进行AASI与ACR和IMT等相关性分析。结果:两组患者年龄、性别组成、血脂水平及BMI指数比较,差异均无统计学意义(P>0.05)。两组AASI、尿白蛋白/肌酐比值、微量白蛋白尿发生率、颈动脉内-中膜厚度及颈动脉异常发生率比较,差异均有统计学意义(P<0.01或P<0.05)。高水平组ACR和微量白蛋白尿发生率高于低水平组(P<0.05)、IMT和颈动脉异常发生率均高于低水平组(P<0.05);AASI与ACR、IMT及微量球蛋白发生率、颈动脉异常率呈正相关(r=0.452,P<0.01;r=0.617,P<0.01;r=0.526,P<0.01;r=0.530,P<0.05)。结论:糖尿病患者AASI变化对预测靶器官损害具有较高临床价值,且操作简便,值得临床推广。
Objective: To investigate the predictive value of dynamic arterial stiffness index ( AASI ) on the target organ damage in patients with diabetes mellitus.nethod: 100 patients with type 2 diabetes who were admitted in our hospital from January 2012 to January 2014 were included in the observation group.The other objects in our hospital during the same period of healthy 100 cases were selected as the control group, AASI and urine protein/ creatinine ratio ( ACR ), carotid artery-media thickness ( IMT ) were respectively measured, and correlation of AASI and ACR and IMT were analyzed.Result: AASI, urinary albumin/creatinine ratio, microalbuminuria incidence, carotid artery-media thickness and abnormal incidence of carotid artery of two groups, there were statistically significant (P〈0.01 or P〈0.05 ) .AASI≤0.51 as the low level group, AASI〉0.51 as the high level group.ACR and the high incidence of microalbuminuria in the high level group were higher than those of the low level'group ( P〈0.05 ), IMT and carotid artery abnormalities were higher than those of the low level group (P〈0.05) ; AASI and ACR, incidence, carotid IMT and trace globulin abnormal rate were positively correlated (r=0.452, P〈0.01 ; r=0.617, P〈0.01; r=0.526, P〈0.01 ; r=0.530, P〈0.05 ) .Conclusion: The changes of AASI in patients with diabetes mellitus has a high clinical value for predicting target organ damage, and it is simple and easy to operate, it is worthy of clinical promotion.
