探讨多囊卵巢综合征采用炔雌醇环丙孕酮片联合二甲双胍治疗疗效及其对胰岛素抵抗、激素影响

Clinical efficacy of ethinyl estradiol ring C progesterone tablets combined with metformin in treatment of polycystic ovary syndrome and its influence on insulin resistance and hormone

目的比较炔雌醇环丙孕酮片联合二甲双胍与单纯炔雌醇环丙孕酮片治疗多囊卵巢综合征临床疗效及对激素、胰岛素抵抗影响。方法选取126例多囊卵巢综合征者按照随机数字表法分组为对照组与治疗组,各63例。对照组采用炔雌醇环丙孕酮片治疗,治疗组于对照组治疗基础上采用二甲双胍治疗。2组连续治疗3个月后评价其临床疗效、治疗前、后性激素及胰岛素抵抗变化情况、排卵率、妊娠率、不良反应。结果治疗组总有效率(96.8%)明显高于对照(77.8%,P<0.05)。2组患者治疗后各项性激素、胰岛素抵抗较治疗前明显改善,且治疗组改善较对照组明显(均P<0.05)。治疗组排卵者及妊娠者分别为93.7%、82.5%明显高于对照组74.6%、50.8%(P<0.05)。治疗过程中2组均无不良反应发生。结论炔雌醇环丙孕酮片联合二甲双胍可有效改善多囊卵巢综合征临床症状,促进其排卵及妊娠,从而提高疗效,同时无明显不良反应发生。

Objective To compare the clinical efficacy of ethinyl estradiol ring C progesterone tablets combined with metformin in treatment of polycystic ovary syndrome,and its influence on insulin resistance and hormone. Methods 126 cases of polycystic ovary syndrome patients were randomly divided into control group and treatment group,63 cases in each group. The control group was given ethinyl estradiol ring C progesterone tablets;treatment group added metformin. 3 months after treatment,the clinical curative effect evaluation after treatment were compared,the ovulation rate,pregnancy rate,adverse reactions and changes of sex hormones and insulin resistance changes before and after treatment were also compared.Results The total effective rate was 96. 8%,higher than that of control group（ 77. 8%,P〈0. 05）. After treatment,the effect of the 2 groups of sex hormones,insulin resistance were significantly improved,and the treatment group improved more obvious effect（ P〈0. 05）. The ovulation and pregnancy of treatment group were 59 cases and 52 cases,respectively 93. 7%,82. 5%,higher than that of control group（ P〈0. 05）,the proortion were 74. 6%and 50. 8%,higher than that of control group（ P〈0. 05）. In the course of treatment,no adverse reaction occurred in 2 groups. Conclusion For polycystic ovary syndrome patients,ethinyl estradiol ring C progesterone tablets combined with metformin can effectively improve the clinical symptoms,promote ovulation and pregnancy,and can improve the curative effect,had no obvious adverse reactions.
