摘要 目的:探讨磁敏感加权成像在评估脑卒中方面的临床应用价值。方法:回顾性分析25例单侧大脑中动脉狭窄或闭塞的患者,症状发作后几小时至24小时内。所有25例患者均行常规MR检查,包括T1WI、T2WI、FLAIR、DWI、SWI、PWI、MRA。采用Alberta卒中项目早期CT评分比较个体DWI、SWI、PWI。结果:88.0%(22/25)的患者在SWI序列上出现静脉显著不对称及平均通过时间(MTT)的延长。SWI-ASPECTS与MTT-ASPECTS关联性最好(r=0.796,P=0.000<0.001)。结论:SWI可提供与MTT可比的脑灌注信息,但SWI在指导溶栓治疗方面需要进一步前瞻性研究的评估。 Objective: To study application value of magnetic susceptibility-weighted imaging(SWI) in the evaluation of cerebral stroke. Methods: It's a retrospective analysis of 25 patients with unilateral middle cerebral artery stenosis or occlusion in a few hours to 24 hours after symptom onset. All of 25 cases underwent routine MR examination, including T2 WI,T1WI, FLAIR, DWI, SWI, PWI. The scores of DWI, SWI and PWI was compared by alberta stroke program early CT score.Results: Asymmetric prominent vessels and prolonged mean transit time(MTT) map were found in 88%(22/25) of patients.SWI-ASPECTS and MTT-ASPECTS were associated with the best(r=0.796, P=0.000〈0.001). Conclusions: SWI has the similar ability to prolonged MTT, which can provide information about brain perfusion. But SWI needs further prospective studies in the guidance of thrombolytic therapy.
