摘要
目的在Ⅳ型胶原酶诱导的实验性脑出血模型的基础上,实施立体定向血肿抽吸术并研究抽吸术后血肿周围血浆神经元特异性烯醇化酶(NSE)的变化及意义。方法成年健康Wistar雄性大鼠135只,随机分为假手术组、脑出血组、抽吸治疗组。将Ⅳ型胶原酶注入大鼠尾状核建立大鼠脑出血模型,在形成脑出血10 h后,向血肿腔内注入含有3 000 U尿激酶的溶液5μl来溶解血凝块,2 h后进行立体定向血肿抽吸术干预治疗。应用Bederson四级评分法评价大鼠神经功能缺损程度,免疫组化法及Western blot分别检测NSE蛋白的表达。结果大鼠脑出血后,各个时间点脑出血组、抽吸治疗组血肿周围脑组织NSE蛋白表达明显高于假手术组(P<0.05);脑出血后第1天,抽吸治疗组NSE蛋白表达略高于脑出血组,但差异无统计学意义(P>0.05);脑出血后第3天、第5天抽吸治疗组与脑出血组比较,NSE蛋白表达明显降低(P<0.01)。结论Ⅳ型胶原酶诱导的大鼠脑出血模型是一种稳定、可靠的且能够实施立体定向血肿抽吸术的理想模型。大鼠脑出血后神经行为功能损伤,且血肿周围组织NSE蛋白表达明显增强。早期进行血肿抽吸术可以较好地清除脑内血肿,降低脑组织NSE蛋白的表达,减少脑出血后神经元的阳性细胞数及变性细胞数,从而保护神经组织,明显改善其神经行为功能。
Objective To establish the stereotactic hematoma aspiration and observe the effect and meaning of stereotactic hematoma aspiration on the experimental rat model of intracerebral hemorrhage (ICH) induced by Ⅳ collagenase. Methods The 135 healthy male adult Wistar rats were randomly divided into sham operation group,ICH group and hemorrhage aspiration group. The models of ICH were established by injecting IV collagenase into caudate nucleus of the rats, which were injected with 3 000 U of urokinase in 5 pd to dissolve blood clots on 10 h after ICH and treatment by hematoma aspiration on 2 h after the urokinase was injected. The neurological impairment in rats was evaluated by means of Bederson. The expression of NSE in cortex was detected by the immunohistochemistry and Western Blot. Results The ex- pression of NSE in the peripheral region of hematoma in hematoma aspiration group and ICH group was significantly higher than that in sham operation group at each time point(P 〈 0.05). The expression of NSE in the peripheral region of hematoma in hematoma aspiration group was slightly higher than that of ICH ground one day after ICH(P 〉 0.05 ), but there was no significant difference. Compared with ICH group, the expression of NSE in the peripheral region of hematoma in hemorrhage aspiration group was significantly decreased on 3rd and 5th day after ICH ( P 〈 0.01 ). Conclusion The rat model of ICH induced by Ⅳ collagenase was stable and reliable and ideal model which can be used to implement stereotactic hematoma aspiration. The neurobehavioral function is damaged and the expression of NSE in perihematomal tissue after ICH is increased obviously. The early hematoma aspiration can remove hematoma better, weaken the expression of NSE, reduce the number of neuronal degeneration cells and improve neurobehavioral function significantly after ICH.
