人生回顾干预对癌症终末期患者心理状况的应用研究

The research of life review intervention on psychological status of patients with end-stage cancer application

目的研究人生回顾干预对癌症终末期患者心理状况的改善效果。方法选取癌症终末期患者60例,按入院时间先后的顺序编号,偶数为观察组,奇数为对照组。观察组采用人生回顾方法实施心理干预,对照组按传统照顾模式进行护理,采用癌症患者生活质量问卷、汉密顿抑郁量表、汉密顿焦虑量表、罗森伯格自尊量表测量的方法评价2组患者干预前后的心理状况,采用t检验方法比较2组患者生活质量的评分差异。结果干预前观察组和对照组患者生命质量评估的各个维度评分以及患者的抑郁、焦虑、自卑情绪评分的差异无统计学意义(P>0.05)。干预后,观察组的躯体功能、角色功能、情绪功能、认知能力、社会功能、整体生活质量的评分均高于对照组,经t检验,t值分别为4.624、7.559、9.557、12.391、14.341、9.135,差异均有统计学意义(P<0.05);干预后观察组的HAMD、HAMA、SES评分均显著低于对照组,t值分别为0.451、0.356、0.491,差异有统计学意义(P<0.05);干预后抑郁、焦虑、自卑发生率为10.0%、16.7%、10.0%,显著低于对照组的56.7%、53.3%、50.0%,差异有统计学意义(P<0.05)。结论人生回顾能提高癌症终末期患者的生活质量,改善癌症患者焦虑、抑郁、自卑的负面心理情绪。

Objective To study the life review intervention, and to improve the psychological status of patients with endstage cancer effect. Methods Sixty cases of end-stage cancer patients were selected in hospital according to time sequence number, even as the observation group, odd for the control group. Observation group life review method is used to implement the psychological intervention and control group according to the traditional nursing care model, USES the cancer patients quality of life questionnaire, Hamilton depression scale, Hamilton anxiety scale, rosenberg self-esteem scale measurement method for evaluation of two groups of patients before and after the intervention of psychological status. Results The intervention of the observation group and control group patients before the assessment of quality of life scores of each dimension, and the patient＇ s score difference of depression, anxiety, inferiority complex has no statistical significance （P 〉 0.05 ）. After the intervention, the observation group of physical function, role function, emotional function, cognitive ability, social function, the overall quality of life score were higher than the control group, the t test analysis showed that the differences were statistically significant （ P 〈 0.05 ） ; After the intervention group of HAMD, HAMA, SES score were significantly lower than the control group, and the difference was statistically significant （P 〈 0.05 ）. After intervention, the ratio of depression,anxiety and inferiority in observation group were 10%, 16.7% and 10% ,lower than those of in control group（P 〈 0.05 ）. Conclusion Life review can improve the quality of life of cancer patients with end-stage, improve cancer patients negative psychological mood, anxiety, depression, inferiority complex, has clinical value.
