隐源性脑脓肿老年患者的临床特征及治疗

Clinical features and treatment of elderly patients with cryptogenic brain abscess

目的总结老年隐源性脑脓肿患者临床表现的特点和治疗。方法回顾性分析17例老年隐源性脑脓肿患者的临床资料。结果采用药物保守治疗2例,手术治疗15例。手术病例中,穿刺引流11例,脓肿切除4例。治愈13例,死亡1例,肢体偏瘫2例,失语1例。结论老年隐源性脑脓肿患者的起病隐匿,症状往往不典型,容易漏诊、误诊,需与颅内肿瘤、脑寄生虫等病相鉴别。明确诊断后应积极、规范足疗治疗;治疗中要注重基础病的控制,减少并发症的发生。

Objective To summarize the features and treatment of elderly patients with cryptogenic brain abscess.Methods The data of 17 elderly patients with implicit source brain abscess were retrospectively analyzed.Results Of 17 cases,2cases were treated conservatively with drugs and 15 cases underwent surgical treatment,in whom the puncture drainage was performed in 11 cases,abscess excision in 4cases.Thirteen cases were cured,1case died,2cases were with limb hemiplegia and 1case was with aphasia.Conclusion The implicit source brain abscess in the elderly has the features of conceality and atypical symptoms and is easily misdiagnosed.Attention should be paid to treating basic disease and reducing the incidence of complications in the treatment of elderly patients with cryptogenic brain abscess.
