腹腔镜下宫颈癌根治术与开腹手术治疗早期宫颈癌的效果对比

Comparison of the Efficacy of Laparoscopic Radical Resection of Cervical Carcinoma and Open Operation in the Treatment of Early Cervical Cancer

目的:对比早期宫颈癌进行腹腔镜下根治术和开腹根治性子宫切除术的疗效,探讨腹腔镜手术治疗早期宫颈癌的优势。方法:选取笔者所在医院诊断早期宫颈癌并行手术治疗的患者82例,随机进行开腹手术42例、腹腔镜手术40例,比较两组手术时间、出血量、清扫淋巴结数目、术后排气时间、住院时间,以及并发症如血管损伤、膀胱尿管肠道损伤、尿潴留、淋巴囊肿、切口感染等的差异。结果:两组手术时间、清除淋巴结数目比较差异无统计学意义(P>0.05);但腹腔镜组出血量、术后排气时间、住院时间明显少于开腹组,差异有统计学意义(P<0.05)。腹腔镜组切口感染率较开腹组低,差异有统计学意义(P<0.05);其他如血管损伤、膀胱尿管肠道损伤、尿潴留、淋巴囊肿发生率两组比较差异无统计学意义(P>0.05)。结论:腹腔镜下宫颈癌根治术和开腹根治性子宫切除术相比,具有创伤小、出血少、恢复快、并发症少、住院时间短等优点,有明显优势和重要的临床应用价值。

Objective：To compare the efficacy of laparoscopic radical hysterectomy and abdominal radical hysterectomy in the treatment of cervical cancer,and to discuss the advantages of laparoscopic surgery for early cervical cancer.Method：In our hospital,82 cases with early cervical cancer who underwent operation were selected,they were randomly divided into two groups,42 cases were treated with laparotomy,40 cases were treated with laparoscopic surgery,the operation time,bleeding volume,dissection of lymph nodes,postoperative exhaust time,hospitalization time and complications such as vascular injury,bladder catheter intestinal injury,retention of urine,lymphatic cyst,infection of incisional wound between the two groups were compared.Result：There were no significant differences between the two groups in the operation time and the number of lymph nodes（P〉0.05）,the amount of bleeding,postoperative exhaust time,hospitalization time in the laparoscopic group were significantly less than those in the laparotomy group（P〈0.05）.The infection of incisional wound in the laparoscopic group was significantly lower than that in laparotomy group（P〈0.05）,and there were no significant differences in the incidence of other complications,such as vascular injury,urinary retention,bladder catheter intestinal injury and lymphatic cyst（P〉0.05）.Conclusion：Laparoscopic radical hysterectomy with less trauma,less bleeding,fewer complications,less complications,shorter hospital stay,and other advantages compares with open radical hysterectomy,it has obvious advantages and important clinical application value.
