Panretinal photocoagulation for diabetic retinopathy

摘要 目的探讨577am激光广泛视网膜光凝术(panretinal photocoagulation)治疗糖尿病视网膜病变的临床效果。方法增生前期糖尿病视网膜病变(PPDR)和增生性糖尿病视网膜病变(PDR)共126例(203眼),接受577am激光广泛视网膜光凝。光凝前后行视力、散瞳后眼底、荧光素眼底血管造影检查。结果广泛视网膜光凝术后随访12个月,PPDR组128眼中视力提高和不变者112眼,占87.50%,3眼因出现玻璃体积血难以吸收而接受玻璃体切除手术。PDR组75眼中视力提高和不变者42眼,占56.oo%,5眼因玻璃体积血难以吸收或纤维增生行玻璃体手术。结论577nm激光广泛视网膜光凝术治疗糖尿病视网膜病变的疗效确切。 Objective To investigate the therapeutic effects of panretinal photocoagulation (PRP) with 577 nm laser for diabetic retinopathy (DR). Methods A total of 203 eyes of 126 cases with preproliferative diabetic retinopathy (PPDR) or proliferative diabetic retinopathy (PDR) were treated with PRP. Visual acuity, fundus examination and fundus fluorescein angiography (FFA) were performed before and after the PRP. Results During the 12-month follow-up after the PRP, among 128 eyes of PPDR the eyes with improved or unchanged visual acuity accounted for 87.50% and 3 eyes were treated with vitrectomy due to unabsorbable vitreous haemorrhage. Among 75 eyes of PDR, the eyes of which the visual acuity improved or didn ' t change accounted for 56. 00%, and 5 eyes were treated with vitrectomy due to unabsorbable vitreous haemorrhage or vitreous fiber proliferation. Conclusion Panretinal photocoagulation with 577 nm laser is effective in the treatment of DR.
