术毕前房注地塞米松对白内障患者视觉质量及屈光状态的影响

【摘要】目的探讨白内障患者在人工晶状体植入术毕前房注射地塞米松的疗效，分析前房注射地塞米松是否对术后患眼视觉质量和屈光状态产生影响。方法选取2013年6月至2014年6月白内障患者100例（100只眼），随机分为观察组与对照组。对照组行人工晶状体植入术，观察组在对照组基础上于术后前房注入地塞米松0．5mg。观察两组患者术后角膜水肿和前房渗出情况；测定两组患者术后视力和不同空间频率下的对比敏感度（CSF）；测定患者手术前后眼轴长度和角膜曲度，分别记录屈光度数的预测值和实测值。结果观察组患者前房渗出和角膜水肿情况明显优于对照组（P〈0.05）；两组患者术后视力情况及CSF值差异无统计学意义（P〉0．05）；两组患者眼轴长度和屈光度数手术前后及组间比较差异均无统计学意义（P〉0．05）。结论白内障患者术毕前房注入地塞米松不会对手术后患眼的视觉质量和屈光状态产生影响，同时还可以降低术后并发眼部炎症的概率，值得临床推广。

Objective To investigate the effect of dexamethasone injectionafter surgery in the treatment of intraocular lens implantation in cataract patients, and to analysis the effect of anterior chamber injection of dexamethasone on postoperative visual quality and refractive status. Method 100 patients with 100eyestreated in our hospital from June 2013 to June 2014 were selected and completely randomly divided into experimental group and control group. Two groups of patients were treated with same intraocular lens implantation.The only difference between the experimental group and the control group was that experimental group were injected 0.Smg dexamethasone after the surgery, and the control group had no injection.The corneal edema and anterior chamber exudation were observed in the two groups.The visual acuity and the contrast sensitivity of two groups of patients were determined.The axial length and corneal curvature was detected before and after the surgery.The predictive value and the measured value of the refractive power are recorded. Results The anterior chamber exudation and corneal edema were significantly better in the experimental group than in the control group （ P〈0.05 ） .There was no significant difference in visual acuity and CSF value between the two groups after the surgery （ P〈0.05 ） . There were no significant differences in the axial length and refractive power no matter it was between the two groups of patients or in the same group of patients but before and after the surgery （ P〈0.05 ） . Conclusion Dexamethasone injectionafter surgery in the treatment of intraocular lens implantation has no negative effect on postoperative visual quality and refractive status.At the same time, it can also reduce the probability of postoperative ocular inflammation, it is worthy of clinical promotion.
