自由体位与传统卧位对正常初产妇产程中产科质量和母婴并发症的影响

【摘要】目的分析自由体位和传统卧位对正常初产妇在产程中产科质量和母婴并发症的影响。方法正常初产妇120例，随机分为观察组和对照组，对照组采用传统卧位待产，观察组产妇第二产程采用自由体位，分析不同体位对产程质量和母婴并发症的影响。结果观察组产妇第二产程时间（41．22±12．37）min显著低于对照组（56．84±16．37）min；产妇分娩比例（86．7％）显著高于对照组（70．O％）；剖宫产比例（5．0％）显著低于对照组（16．7％），差异均有统计学意义（P〈0．05）。两组产妇产后大出血差异无统计学意义（P〉0．05），观察组产后不适、新生几窒息、会阴侧切显著低于对照组（P〈0．05）。结论产妇在第二产程采用自由体位分娩能够缩短第二产程时间，提高产科质量，减少母婴并发症，具有推广价值。

Objective To analysze the effect of quality of obstetric and matemal complications with freedom and traditional supine position in normal primipara.Methods 120 normal primipara in hospital obstetrics from June 2014 to May 2015 were selected, primipara were randomly divided into study group and control group, study group patients were taken delivery of free position, control group patients were taken delivery of traditional supine be produced, The quality of obstetric and maternal complications of normal primipara were compared. Results The second stage of labor time study group （ 41.22 ± 12.37 ） min was significantly lower than the control group （ 56.84 ± 16.37 ） rain, P〈0.05. Natural childbirth proportion of the study group （ 86.7% ） was significantly higher than the control group （ 70.0% ） , P〈0.05, the proportion of caesarean section study group （ 5.0% ） was significantly lower than the control group （ 16.7% ） , P〈0.05. The postpartum hemorrhage of the two groups of was significantly difference, P〉0.05. The postpartum hemorrhage and postpartum discomfort proportion in study group were significantly lower than the control group, P〈0.05, the asphyxia proportion in study group （ 3.3% ） was significantly lower than the control group （ 13.3% ） , P〈0.05. Thg episiotomy proportion of study group （ 48.3% ） was significantly lower than the control group （ 66.7% ） , _0〈0.05. Conclusion Mothers in the second stage of labor using the free position and a second generation can shorten delivery time, improve the quality of obstetric and reduce maternal complications, which has promotional values.
