雷珠单抗治疗糖尿病性黄斑水肿的疗效观察

Observation of clinical effect of lucentis in treatment of diabetic macular edema

目的对玻璃体腔内注射雷珠单抗治疗糖尿病性黄斑水肿的疗效进行评价。方法选取自2013年1月~2014年1月来我院住院治疗的糖尿病性黄斑水肿患者80例作为研究对象,随机分为对照组和观察组,每组各40例,两组患者均行玻璃体腔内注射,观察组同时注入雷珠单抗（Lucentis）0.5mg（0.05m L）,对照组注入曲安奈德4mg（0.1m L）,比较两组的治疗疗效,以及分别记录治疗前后患者的最佳矫正视力（BCVA）的变化情况。结果观察组治疗后的总有效率为95.0%;对照组治疗后的总有效率为75.0%。观察组治疗后的疗效显著优于对照组,两组比较差异有统计学意义（P〈0.05）。治疗前两组患者的最佳矫正视力（BCVA）比较,差异不显著,治疗后2、12周,两组患者的最佳矫正视力（BCVA）分别较治疗前显著改善,且观察组患者治疗后2、12周的最佳矫正视力（BCVA）显著优于对照组,两组比较差异有统计学意义（P〈0.05）。结论玻璃体腔内注射雷珠单抗治疗糖尿病性黄斑水肿可以提高疗效,显著改善患者的视力,值得推广和应用。

Objective To evaluate the curative effect of intravitreous injection of lucentis in treatment of diabetic macular edema. Methods 80 patients with diabetic macular edema who were hospitalized and treated in our hospital from January 2013 to January 2014 were selected as research objects and randomly allocated to the control group and the observation group, with 40 in each. Patients were all received intravitreous injection. The observation group was injected with 0.5mg（0.05 m L） lucentis while the control group was injected with 4mg triamcinolone acetonide（0.1m L）. Curative effects of two groups were compared. Changes in best corrected visual acuity（BCVA） before and after treatment of patients were recorded. Results After treatment, total effective rates of the observation group and the control group were 95.0% and 75.0% respectively. The curative effect after treatment of the observation group was significantly better than that of the control group. Comparative difference of two groups had statistical significance（P〈0.05）. Best corrected visual acuity（BCVA） of 2 weeks and 12 weeks after treatment of two groups were both significantly improved than that before treatment. And best corrected visual acuity（BCVA） of 2 weeks and 12 weeks after treatment of the observation group was significantly better than that of the control group. Comparative difference of two groups had statistical significance（P〈0.05）. Conclusion Intravitreous injection of lucentis in treatment of diabetic macular edema can significantly improve curative effect and improve visual acuity of patients, worthy of application and promotion.
