膀胱部分切除术在肌层浸润性膀胱癌治疗中的价值

Value of partial cystectomy in treatment of muscular invasive bladder cancer

目的探讨膀胱部分切除术治疗肌层浸润性膀胱癌的临床疗效。方法选取2013年1月~2014年12月期间在我院住院治疗的肌层浸润性膀胱癌患者85例作为研究对象。随机分为实验组（45例）和对照组（40例）,实验组患者给予膀胱部分切除术进行治疗,对照组患者给予根治性膀胱全切术进行治疗,观察并对比两组患者的复发情况、并发症发生情况、手术时间、留置导尿管时间、住院时间以及术中出血量等指标。结果实验组患者的复发率为31.1%,对照组患者的复发率为37.5%,两组患者比较差异无统计学意义（P〉0.05）。实验组患者并发症的发生率为24.4%,对照组患者并发症的发生率为45.0%,两组比较差异有统计学意义（P〈0.05）。实验组患者的手术时间、留置导尿管时间、住院时间以及术中出血量等均明显小于对照组患者,两组患者比较差异有统计学意义（P〈0.05）。结论膀胱部分切除术治疗肌层浸润性膀胱癌并发症的发生率较低,术后不易复发,疗效显著,具有一定的临床应用价值。

Objective To explore the clinical curative effect of partial cystectomy in treatment of muscular invasive bladder cancer. Methods 85 patients with muscular invasive bladder cancer who were hospitalized in our hospital from January 2013 to December 2014 were selected as research objects and they were randomly allocated to the experimental group（45 cases）and the control group（40 cases）.Patients in the experiment group were given partial cystectomy while patients in the control group were given radical cystectomy.Recurrence,incidence of complications, operative time, indwelling catheter time, hospitalization stay, blood loss and other indicators of patients in two groups were observed and compared. Results The recurrence rates of the experimental group and the control group were 31.1% and 37.5% respectively.Comparative difference between two groups had no statistical significance（P〈0.05）.Incidences of complications of the experimental group and the control group were 24.4% and 45.0% respectively.Comparative difference between two groups had statistical significance（P〈0.05）.Operative time, indwelling catheter time,hospital stay and the blood loss of the experimental group were significantly less than those of the control group. The difference between two groups were statistically significant（P〈0.05）. Conclusion Partial cystectomy in treatment of muscular invasive bladder cancer has low incidence of complications and low rate of postoperative recurrence rate.In addition,it has a significant curative effect and it is of certain clinical applicative value.
