单纯修补术与胃大部分切除术治疗急性胃穿孔疗效比较

Comparison on effect of simple repair operation and subtotal distal gastrectomy operation treatment of acute gastric perforation

目的分析急性胃穿孔采用不同手术治疗方案的临床效果。方法抽选我院2005年6月~2014年6月期间收治的患者90例作为研究对象,按照不同手术方法分为两个组别,每组各45例。其中对照组实施单纯胃修补术,试验组实施胃大部分切除术,观察手术治疗指标,比较术后并发症和复发情况。结果试验组患者术中出血量多,手术用时、住院时间长,和对照组比较差异显著。试验组并发症发生3例（6.7%）,穿孔复发1例（2.2%）;对照组并发症发生10例（22.2%）,穿孔复发7例（15.6%）,差异有统计学意义（P〈0.05）。结论对于急性胃穿孔患者而言,单纯胃修补术和胃大部分切除术均有良好效果。其中前者治疗恢复时间短,后者并发症和复发率低,应该根据患者的实际情况进行合理选择。

Objective To analyze the clinical effect of different operation treatment methods on acute gastric perforation. Methods 90 patients with acute gastric perforation, who were received and treated in our hospital from June 2005 to June 2014, were selected as the reseach objects and were randomly divided into two groups according to differet operation methods, with 45 cases in each group. Thereinto, to implement simple stomach repair operation on control group, to implement subtotal distal gastrectomy operation on test group, to observe the index of operation treatment, to compare the complications and recurrence condition after operation. Results The intraoperative blood loss was more, the operation time and hospitalization time were longer of patients in test group, the differences was statically significant compared with which in control group. The incidence of complications and the perforation recurrence rate in test group[3 cases（6.7%）, 1 case（2.2%）] were statically significant compared with which in control group[10 cases（22.2%）, 7 cases（15.6%）]（P〈 0.05）. Conclusion Simple stomach repair operation and subtotal distal gastrectomy operation in the treatment of acute gastric perforation all have better effect. Thereinto, the treatment recovery time of the former is shorter, while the complications and the recurrence rate of the latter were lower, the operation methods should be reasonable selected according to the actual situation of patients.
