苯巴比妥与高压氧在新生儿缺氧缺血性脑病中的应用

The Application for Phenobarbital and Hyperbaric Oxygen in Neonatal Hypoxic Ischemic Encephalopathy

目的:研究苯巴比妥及高压氧联合应用于新生儿缺氧缺血性脑病的临床疗效。方法:选取2014年4月-2015年4月本院收治的88例新生儿缺氧缺血性脑病患者作为研究对象,根据治疗方法不同分为对照组与联合组,每组各44例。对照组给予对症支持等常规方法进行治疗,联合组在常规治疗基础上再行苯巴比妥及高压氧联合治疗,观察两组的临床疗效。结果:联合组治疗总有效率为79.5%,明显高于对照组的47.7%,比较差异有统计学意义(P<0.05);联合组治疗28 d后,脑电图异常6例,明显低于对照组的15例,比较差异有统计学意义(P<0.05);治疗前联合组及对照组NBNA评分比较差异无统计学意义(P>0.05),治疗1个疗程后,联合组NBNA评分高于对照组,比较差异有统计学意义(P<0.05);联合组在意识障碍消失时间、原始反射恢复时间、四肢肌张力恢复时间等方面均优于对照组,比较差异均有统计学意义(P<0.05)。结论:采用苯巴比妥及高压氧联合治疗新生儿缺氧缺血性脑病患者能够取得良好疗效,具有较高的安全性及可靠性。

Objective：To study the curative effect of Phenobarbital and hyperbaric oxygen during the clinical treatment of neonatal hypoxic ischemic encephalopathy.Method：A total of 88 cases with neonatal hypoxic ischemic encephalopathy in our hospital from April 2014 to April 2015 were selected as the research objects,they were divided into the control group and the combination group according to the differences of treatment methods,each group had 44 cases.The control group was given symptomatic support and other conventional methods of treatment,while the combination group combined treatment of Phenobarbital and hyperbaric oxygen on the basis of routine treatment,the clinical efficacy of the two groups were observed.Result：The total effective rate of combination group was 79.5%,significantly higher than the control group 47.7%,the difference was statistically significant（P〈0.05）.The combination group had 6 cases of abnormal EEG after 28 d treatment,lower than that of 15 cases in control group,the difference was statistically significant（P〈0.05）.Before treatment,the combination group and control group NBNA score had no significant difference（P〉0.05）,after 1 courses of treatment,the combination group NBNA score was higher than the control group,the difference was statistically significant（P〈0.05）.the combination group in lost consciousness time,primitive reflex recovery time and recovery time of muscle tension were better than the control group,the differences were statistically significant（P〈0.05）.Conclusion：The patients were treated with Phenobarbital and hyperbaric oxygen in neonatal hypoxic ischemic encephalopathy can achieve good curative effect,and with high safety and reliability.
