绒毛细胞核型分析在胚胎停育中的应用观察 被引量:1

摘要 目的:研究胚胎停育的原因并探讨绒毛细胞核型分析在妊娠指导及胚胎停育中的应用价值。方法:选取2014年1月-2015年12月本院收治的胚胎停育患者手术清宫绒毛组织标本100例,作为研究对象。对所有无菌绒毛标本进行长期细胞培养、染色体制备与细胞核型分析,同时分析患者胚胎停育产生的原因,研究绒毛细胞核型在妊娠指导及胚胎停育中的应用价值。结果:100例胚胎停育绒毛组织标本经长期细胞培养成功95例,培养成功率达95.00%,培养成功标本中检出异常核型34例,异常率为35.79%,异常核型分类中常染色体三体标本16例(47.06%),三倍体标本7例(20.59%),四倍体标本3例(8.82%),嵌合体标本3例(8.82%),性染色体单体标本3例(8.82%),结构异常2例(5.88%),其中常染色体三体异常检出率明显高于其他异常核型检出率,差异有统计学意义(P<0.05);绒毛细胞核型正常标本61例中,男性核型25例(40.98%),女性核型36例(59.02%),性别核型构成比比较差异有统计学意义(P<0.05)。结论:胚胎绒毛细胞核型异常是导致胚胎停育的主要原因之一,并且异常核型以常染色体三体最为常见,通过分析绒毛染色体核型有助于明确患者胚胎停育的原因,可对患者下次妊娠提供合理的指导,具有临床应用及推广价值。 Objective:To study the reasons of embryo damage and discuss the application value of hair cells karyotype in the embryo damage and pregnancy guidance.Method:From January 2014 to December 2015,100 cases of embryo damage patients' curettage villi tissue samples,which admitted in our hospital were selected as the research objects. All sterile villi samples were given the long-term cell culture,chromosome preparation and karyotype analysis.The cause of patients with embryo damage was analyzed at the same time.The application value of hair cells karyotype analysis in the embryo damage and pregnancy guidance was analyzed.Result:A total of 100 cases of embryonic damage villi tissue samples by cell culture for a long time,95 cases of success,the success rate was 95.00%.Development successful specimens' detected abnormal karyotype in 34 cases,the abnormal rate was 35.79%,in the abnormal karyotype classification, triplox specimens 16 cases(47.06%),triploid specimens 7 cases(20.59%),tetraploid specimens 3 cases(8.82%),chimeric specimens 3 cases(8.82%),sex chromosome single specimens 3 cases(8.82%),abnormal structure 2 cases(5.88%).Abnormal autosomal triplox detection rate was much higher than other abnormal karyotype detection rate,the difference was significant statistically significant(P〈0.05).For the 61 cases hair cells karyotype normal specimens,the male karyotype 25 cases(40.98%),female karyotype 36 cases(59.02%),sex karyotype composition had more significant differences,the difference was significant statistically significant(P〈0.05).Conclusion:Embryo villus cell abnormal karyotype is one of the main cause of the embryonic damage,and abnormal karyotype autosomal triplox is the most common,through the analysis of hair cells karyotype will help to clear the reason of embryonic damage patients,which can provide reasonable guidance on the next pregnancy patients and has clinical application and popularization value.
