腹腔镜修补术治疗胃、十二指肠溃疡穿孔的临床效果观察

Clinical effects of laparoscopic repair surgery on perforated gastroduodenal ulcer

目的探讨腹腔镜胃、十二指肠溃疡穿孔修补的手术效果。方法将2010年10月-2014年10月在本院行腹腔镜胃、十二指肠溃疡穿孔修补30例患者设为观察组,另选择2006年10月-2010年9月行开腹胃、十二指肠溃疡穿孔修补术的31例患者作为对照组,观察两组患者手术时间、术中出血量、术后首次排气时间、住院时间、术后并发症及2个月后复查胃镜溃疡是否愈合。结果观察组患者手术时间、术后排气时间、住院时间短于对照组（P〈0.05）,术中出血量明显少于对照组（P〈0.05）;观察组并发症发生率（6.7%）明显低于对照组（22.6%）（P〈0.05）;观察组溃疡愈合30例,对照组溃疡愈合30例,两组比较差异无统计学意义（P〉0.05）。结论腹腔镜胃、十二指肠溃疡穿孔修补术,创伤小,术后并发症少,恢复快,且集诊断及治疗于一体,值得临床推广应用。

Objective To investigate the effects of laparoscopic repair surgery on perforated gastroduedenal ulcer.Methods A total of 30 patients undergoing laparoscopic repair surgery for perforated gastroduodenal ulcer in our hospital from October 2010 to October 2014 were selected as the observation group,and a total of 31 patients undergoing open repair surgery for perforated gastroduedenal ulcer from October 2006 to September 2010 were selected as the control group.The operation time,intraoperative bleeding volume,postoperative first exhaust time,hospitalization time,postoperative complications in the two groups was observed and 2 months after the review of endoscopic ulcer healing. Results The duration of operation,postoperative exhaust time and length of hospital stay was shorter in the observation group than that in the control group respectively（P〈0.05）,the intraoperative blood loss of the observation group was significantly lower than that of the control group（P 0.05）,and the complication incidence of the observation group was6.7%,which was significantly lower than 22.6% of the control group（P〈0.05）;the gastroscopy 2 months after operation observed 30 cured cases in the observation group and 30 cured cases in the control group,and the difference was not statistically significant（P〉0.05）. Conclusion Laparoscopic repair surgery for perforated gastroduedenal ulcer has advantages of minimal invasiveness,less postoperative complications and rapid recovery,with both diagnosis and treatment functions,and thus is worthy of wide clinical application.
