PICC与留置针在重症急性胰腺炎患者中的应用效果比较

Comparison the application effects of PICC and venous indwelling needle in severe acute pancreatitis

目的比较经外周静脉置入中心静脉导管（PICC）与静脉留置针在重症急性胰腺炎患者中的应用效果,探讨如何为重症急性胰腺炎患者提供安全、快速的静脉给药途径。方法选取2014年1-12月我科收治的60例重症急性胰腺炎患者,随机分为实验组（PICC组）32例和对照组（留置针组）28例,比较两组患者的并发症发生率、留置时间、材料费用及护理满意度。结果实验组并发症发生率为6.3%,明显低于对照组的71.4%,差异有统计学意义（P〈0.01）。实验组患者PICC留置时间为（378.00±175.73）h,材料费用为（1680.00±0.00）元,实验组留置时间明显长于对照组（40.44±19.04）h,材料费用高于对照组（141.82±131.37）元,差异有统计学意义（P〈0.01）,满意度比较差异有统计学意义（P〈0.01）,患者的日常生活活动不受限制,痛苦少,满意度高。结论在重症急性胰腺炎患者输液治疗中,PICC置管效果明显优于留置针,值得临床推广。

Objective To compare the application effects of PICC and venous indwelling needle in severe acute pancreatitis and to explore how to provide safe and fast intravenous drug delivery way for SAP patients. Methods 60 SAP patients from January 2014 to December of our department were selected and were randomly divided into experimental group（PICC group,n=32） and control group（indwelling needle group,n=28）.The incidence rate of complications,indwelling time,material cost and nursing satisfaction in two groups were compared. Results The incidence rate of complications in experimental group was 6.3%,obvious lower than that was 71.4% in control group,and the difference was statistical significane（P〈0.01）.The PICC indwelling time and material cost in experimental group were respectively（378.00±175.73）h and（1680.00±0.00）yuan,obvious longer or higher than that were respectively（40.44±19.04） h and（141.82±131.37） yuan in control group,and the difference was statistical significane（P〈0.01）.The difference of satisfaction had statistical significance between two groups（P〈0.01）.The patient′s daily life activities is not restricted,with less painful and high satisfaction. Conclusion Duing the transfusion treatment of SAP patients,the indwelling catheter effects of PICC was obvious better than indwelling needle,and is worth to popularize.
